Study On Nursing Intervention About The Quality Of Life In Patients With Laryngeal Carcinoma After Larygectomy

Objective To explore the effect of nursing intervention about the quality of life inpatients with laryngeal carcinoma after larygectomy，evaluate the prospect of clinicalapplication to draw up nursing intervention model. Methods Select The FirstAffiliated Hospital of Bengbu Medical College，Department of Otorhinolaryngologypatients for study, The patients with laryngeal carcinoma conformed by patholoy inOctober2008to May2010and experienced a partial larygectomy or totallarygectomy, there are85patients were studied, the experimental group was43, thecontrol group was42.The two group were offered conventional nursing care，Whilethe experimental group was，on the basis of conventional nursing，offered a6-monthdirected intervention. By means of Functional Assessment of Cancer Therapy Headand Neck(FACT-H&N) and self-care skills with pipe quality of life scale，anassessment was made after intervention was finished. Results1.nursing interventioncan significantly improve the overall quality of life with laryngeal carcinoma afterlarygectomy（P=.000）,especially for the partial larygectomy（P=.010）,while the effectfor total larygectomy is little（P=.062）.2.Nursing intervention can improve theself-care skills in patients with pipe，especially in self-touching the neck lymph nodes（P=.002）、thinning sputum to keep smooth of inner tube（P=.004）、cover Stomawhen out（P=.027）,experimental is evidently higher than control group，there issignificant difference.3.To analyze the effect factor by logist regression in patientswith laryngeal carcinoma after larygectomy，variables in the equation is：nursingintervention（B=-3.465，OR=.031，P=.000）,with pipe（B=2.279，OR=9.771，P=.004）,radiotherapy（B=2.147，OR=8.559，P=.035）, complications（B=1.401，OR=4.061， P=.049）, that is the main factors after larygectomy. Conclusion1.nursingintervention can significantly improve the overall quality of life with laryngealcarcinoma after larygectomy（P<0.05）, especially for the partial larygectomy，that:the intervention is effective, it is benefit for patients to return society earlier.2.nursing intervention can improve self-care skills in patients with laryngeal carcinomaafter larygectomy with pipe.3.The main factors that effect the quality of life inpatients with laryngeal carcinoma after larygectomy is nursing intervention、withpipe、radiotherapy、complications.
