Objective: The research is aimed at the impact of the risk factors about postoperativecomplications after hilar cholangiocarcinoma, in order to assess the risks of surgery andpostoperative complications, and optimize clinical decision-making from a certainreference.Methods: Two hundreds and forty-five patients with hilar cholangiocarcinoma from2002.1to2011.10after operation were reviewed retrospectively and analyze the riskfactors of postoperative complications.151male patients and94female patients with theage of28to80years were included in this research, the average age of57.27years,median age58years, male to female ratio1.6:1. Ten factors including sex, age,preoperative jaundice, preoperative albumin,accompanied diseases, combined liverresection or not, preoperative biliary drainage, preoperative hemoglobin, intraoperativeblood loss, vascular invasion (portal vein/hepatic artery) were chosen as risk factors.P<0.05was considered statistically significant.Results: In the research, postoperative complications occurred in78of245patients(31.8%).Three cases of deaths, respectively, died after4,5,26days. Single factorChi-square test showed that6factors might influence occurrence of the postoperativecomplication, including age(P=0.037), preoperative jaundice(P=0.016), preoperativeablumin(P=0.001),combined liver resection or not(P=0.001),,intraoperative bloodloss (P=0.042). In multivariate analysis, however, results showed that4factorsinfluenced occurrence of the postoperative complication,including age(OR=1.823),preoperative jaundice(OR=2.207),preoperative ablumin(OR=0.305) and combinedliver resection(OR=2.774).With the increasing of the patient’s age, the degree ofpreoperative jaundice and combined liver resection, decreasing of preoperative albumin,the risks of postoperative complications would rise.Conclusions: Four factors influenced occurrence of the postoperativecomplication,including age,preoperative jaundice,preoperative albumin and combinedliver resection.
