Objective:To investigate the clinical characteristics of hilar cholangiocarcinoma,the choice of different types of surgical treatment,analyze the risk factors for early postoperative complications,and summarize the clinical experience of diagnosis and treatmentMethods:The clinical data of 120 cases of hilar cholangiocarcinoma treated in our hospital from January 2011 to June 2016 were retrospectively analyzed.According to the clinical data of Bisumth classification,patients was classified type ?,?,? and?,each type was divided into different treatment groups according to different treatment,the survival time and survival rate of different treatment groups were observed,and the risk factors of postoperative complications in surgical treatment group were analyzed.Results:?The general data of different types of patients included gender,age,weight,clinical manifestations,preoperative complications and other aspects were not statistically significant differences.(P>0.05)?For type ? and ? patients,the mean survival time and survival rate after operation for 1 year and 2 years were better than that of other treatments(Palliative resection,PTCD,Biliary stent placement).(P<0.05)?The mean survival time after Liver/extended hepatectomy in type ?patients and the survival rate of 1 year and 2 years were better than that of Bile duct-jejunum anastom-osis,PTCD and Biliary stent placement,the difference was statistic-ally significant.(P<0.05)?For type iv patients,the mean survival time after and a half/extended hepatectomy and biliary stent placement was better than that of bile duct resection,liver resection,surgical biopsy,PTCD drainage and biliary stent placement,but there was no statistically difference in the survival rate of 1 year and 2 years after operation,.(P>0.05)?Gender,preoperative jaundice,surgical time,intraoperative bleeding,intraoperative blood transfusion were not related to early postoperative complications,the difference was not statistically significant.(p>0.05),age,operation mode,preoperative liver function,surgical resection range were the risk factors for early postoperative complications(p<0.05).Conclusions:1.The resection of bile duct can improve the prognosis of type ? and type ? patients and improve postoperative survival rate,so it is necessary for patients with type 1 and type ?.2.The mean survival time after and a half/extended hepatectomy in type ? patients and the survival rate of 1 year,2 years and 3 years after operation were better than that of other treatments.3.In improving the prognosis of patients with type ?,there was no significant difference between bile duct resection,and a half resection,PTCD drainage and biliary stent placement.4.Age,surgical modalities,preoperative liver function and surgical resection range are the risk factors for early postoperative complications of hilar cholangiocarcinoma.
