Objective:To investigate the effects of preemptive analgesia with a small doseof ketamine, flurbiprofen axetil or parecoxib sodium on patients undergoinggynecological open operation. Methods:Eighty patients (ASAⅠ~Ⅱ) undergoing elective gynecological openoperation were randomly divided into four groups:control group (group N), ketaminegroup (group K), flurbiprofen axetil and ketamine group (group KF), parecoxibsodium and ketamine group (group KP). All patients were used with generalanesthesia and received patient-controlled intravenous analgesia (PCIA).The patientsin group K, group KF and group KP were injected with0.3mg/kg ketamineintravenously in5min before cutting skin, the patients in group KF receivedintravenous injection of1mg/kg flurbiprofen axetil after tracheal intubation, whilethe patients in group KP received intravenous injection of0.8mg/kg parecoxibsodium. Record the change of SBP,DBP,HR,SpO2at the anesthesia inductionbefore, the anesthesia induction after, cutting skin, the ending of surgery andextubation. The visual analog scale (VAS) was assessed at1h,6h,12h,24h and48h by single blind method after the surgery. The PCIA effective trigger number was recorded and the patients who has adverse reactions such as nausea, vomiting anditching were also recorded. Results:The age, weight, ASA and the times of surgery in each group had nodifference(P>0.05), so did the SBP, DBP, HR, SpO2at each points(P>0.05). Themean VAS and the PCIA effective trigger number in group K, group KF and groupKP were lower than that in N group at1h,6h,12h,24h,48h after surgery (P<0.05).However, no significant difference was found among group K, group KF and groupKP(P>0.05).Conclusions:A small dose of ketamine, flurbiprofen axetil or parecoxib sodiumhas the effects of preemptive analgesia in patients undergoing gynecological openoperation while they has less adverse reactions.
