Crown and bridge restoration needs tooth preparation in order to obtain the space and for insertion and removal path for prosthesis. During abutment preparation a lot of dentinal tubules get exposed that increases the potential of postoperative dentine sensitivity. All will have sensitivity after tooth preparation on vital tooth but not all but few suffer from sensitivity after cementation of crown and bridge. Still, other factors can induce hypersensitivity such as temporary crowns or final restorations of poor quality and marginal micro-leakage. As a result, many desensitizers came and still coming. And a great number of clinical trials have proved that they can effectively reduce the extent of sensitivity. However, reports about the effect of desensitizers-Hybrid coat and Pro-arginine technology based Colgate sensitive pro-relief desensitizing Toothpaste are extremely low and whichever was done mostly are in vitro and the few clinical trial done that was also in different patient with single agent.The purpose of this study was to observe the microscopic characteristics of dentinal tubules after using desensitizers, the clinical efficiency of hybrid coat and Colgate sensitive pro-relief desensitizing toothpaste as desensitizing agent based on age, sex, types of tooth and ethnicity. Similarly, to find out which one is better in same individual.Material and methodsIn Vitro study, Freshly extracted human premolars were prepared with a speed handpiece and the dentinal tubules were exposed. After using Hybrid Coat, Colgate sensitive pro-relief desensitizing toothpaste in corresponding groups, the microscopic characteristics of dentinal tubules were observed with scanning electron microscopyIn Vivo study, Study was performed at the Department of Prosthodontia, Xiangya stomatology hospital, Central south University China and Lumbini Zonal Hospital,Palpa Dental Clinic of Butwal and Universal College of Dental Surgery, Bhairahawa, Nepal. Patients undergoing two vital tooth preparation for crown/bridge were selected for the experimental volunteers and who gave informed consent was included in the study. A visual analog scale (VAS) consisting10cm line with0at one end indicating no pain/discomfort at all and 10cm at the other end indicating extremely painful was used to indicate dentin sensitivity in response to an air stimulus. Participants were asked to draw a vertical line or circle at point that correspond to their reaction to air stimulus. Tooth were randomly tested for sensitivity and test was done by single blind trial.Results:1.In SEM Observation:Hybrid Coat could occlude almost all of the exposed Dentinal tubules while anti-sensitivity Colgate sensitive pro-relief desensitizing toothpaste could only seal the majority dentinal tubules and few of them were left open. Sediments were visible no matter which desensitizer was used.2. In Clinical Observation:The Average VAS scale of sensitivity before the application of Hybrid coat was6.49and after application immediately was1.91and in1week was2.25; Whereas Pro-Argin based toothpaste had6.20,3-93and4.64respectively. Both can reduce sensitivity.3. Hybrid coat and Colgate sensitive pro-relief desensitizing toothpaste are clinically significance to reduce Dentin Sensitivity as P is0.00i.e. P<0.05.But both doesnot have significant variation i.e. P>0.05on Age,Gender, Ethnicity and tooth factors.4. Compare to Colgate sensitive pro-relief desensitizing toothpaste, Hybrid coat has better effect because P is<0.05immediately and till7day Even though procedure is lengthy and tedious..;Conclusion:1. Hybrid Coat and colgate sensitive pro-relief desensitizing paste are able to seal dentinal tubules and both can reduce sensitivity efficiently.2. Hybrid Coat Desensitizing agent has better clinically significant effect on dentin sensitivity over colgate sensitive pro-relief desensitizing paste3. Sensitivity of prepared vital tooth does not varies significantly with age, sex, ethnicity, type and location of tooth.
