Object:To detect Dkkl in gingival crevicular fluid and inflam-matory gingival tissue from chronic periodontitis and healthy control group by enzyme linked immunosorbent assay (ELISA) and immuno-histochemistry, exploring the relationship of Dkkl with the pathogenesis of periodontitis to provide the experimental basis for periodontal regeneration.Methods:1. Selected30patients with chronic periodontitis as the case group,10healthy volunteers as the control group, The tooth of molar46were selected from all subjects. According to periodontal status, the patient group can be divided into four groups:1) slight chronic periodontitis group (10cases)2) moderate chronic periodontitis group (10cases)3) severe chronic periodontitis group (10cases)4) healthy control group (10cases). Observed and recorded the clinical indicators of the selected molar: gingival index (GI), probing depth (PD), clinical attachment loss (CAL). Collected all gingival crevicular fluid of the selected molar with filter paper strip method and detected Dkkl in gingival crevicular fluid by enzyme-linked immunosorbent assay (ELISA) and tested the correlation with GI,PD,CAL.2. Collected the gingival tissue from9healthy people,8gingivitis,12chronic periodontitis patients with a total of29cases. Detected the distribution and content of Dkkl by immunohistochemical method and computer image processing technology.Results:1. The concentration of Dkkl in gingival crevicular fluid of the chronic gingivitis (8.86±4.07ug/L) is significantly higher than that in the control group (5.51±2.05ug/L)(p<0.05). 2. The concentrations of Dkkl in gingival crevicular fluid of slight chronic periodontitis was5.17±1.49ug/L, moderate chronic periodontitis was8.32±1.25ug/L, severe chronic periodontitis was13.07±3.80ug/L The concentrations of Dkkl in gingival crevicular fluid between slight periodontitis, moderate periodontitis, severe periodontitis was signi-ficantly higher, P<0.05.3.There was a significant positive correlation between the concentrations of Dkkl in gingival crevicular fluid of chronic periodon-titis with PD, CAL, correlation coefficient:r=0.769, P=0.002, r=0.802, P=0.001. There was no significant correlation with GI, r=-0.182, P=0.517.4. The Dkkl-positive epithelial cells were detectable both in control and chronic periodontitis group. In connective tissue, there was detectable Dkkl expression in chronic periodontitis group, especially of the endothelial cells, fiberblasts. In contrast, the Dkkl weakly expressed in controls.5. The mean optical density of Dkkl in gingival tissues of chronic periodontitis group was (13.40±2.83)×10-2, gingivitis group was (7.95±4.52)×10-2, the control group was (5.31±1.40)×10-2. The mean optical density of Dkkl of periodontitis group was significantly higher than that of the gingivitis group, normal group (p<0.05);The difference between the latter two groups was not statistically significant (p>0.05).Conclusion:1. The Dkk1was detected in gingival crevicular fluid in both of chronic periodontitis group and control group, which the former one was significantly higher than the latter.2. In chronic periodontitis, with the increasing of the degree of periodontal destruction, the Dkkl concentration tended to increase. 3.The concentration of Dkkl in the gingival crevicular fluid of chronic periodontitis was positively correlated with PD, CAL, which suggestes that Dkkl was closely related with the damage of periodontitis.4. The Dkkl-positive epithelial cells were detectable both in control and chronic periodontitis group, which the former one was significantly higher than the latter.5.Dkkl was probably served as an important indicator of clinical detection of periodontitis...
