Objective:To explore the value of three demension speckle tracking echocardiography in assessing left ventricular global systolic function in patients with essential hypertension.Methods:Thirty EH patients with left ventricular normal geometry (LVN), thirty EH patients with left ventricular remodeling(LVR)and thirty normal controls were enrolled in this study. The apical full-volume was obtained in all subjects using ARTIDA ultrasonic system of toshiba equipped with3DT analysis software. All above images were stored for off-line analysis. All parameters were collected, including global longitudinal strain(GLS), global radial strain(GRS), global circumferential strain(GCS), the peak systolic rotation in basal plane(Prot-B)and apical plane (Prot-A), the peak systolic global rotation(LV-rot)and twist of LV(LV-tw). The above parameters of the LVN and LVR group were compared with those of control group.Results:①Comparisons of clinical data among three groups:Age, height, body surface area, heart rate were not significantly different among the three groups(P>0.05); Compared with the control group, systolic blood pressure and diastolic blood pressure were significantly increased in the LVN and LVR group (P<0.05), but there were not significantly different between LVN group and LVR group(P>0.05).②Compared with the control group, the value of IVSd, LVPWT, RWT, LVMI were increased significantly in LVN and LVR group (P<0.05);Compared with LVN group, the value of IVSd, LVPWT, RWT, LVMI were further increased in LVR group (P<0.05).③Compared with the control group, the value of GLS was significantly reduced in LVN (P<0.05), there was only a decreased trend in GCS, GRS(P>0.05),the value of GLS, GCS, GRS were significantly reduced in LVR group (all P<0.05); Compared with LVN group, the value of GLS, GCS, GRS were further decreased in LVR group(P<0.05). LVEF was also not significantly different among the three groups(P>0.05)④ Compared with the control group, the value of Prot-B, Prot-A, LV-rot, LV-tw were significantly higher in LVN group and LVR group (all P<0.05).Compared with LVN group, the value of Prot-B, Prot-A, LV-rot, LV-tw were further increased (all P<0.05).⑤GLS, GCS, GRS, LV-rot, LV-tw within researchers or between researchers were all less than15%.Conclusion:3D-STE can sensitively reflect the early change of heart function in patients with essential hypertension by measuring the left ventricular global strain, global rotation and global twist of left ventricle.
