| Objective: Application of three-dimensional ultrasound speckletracking technologies (3D-STE)) evaluation of chronic kidney disease(uremia) in patients with left ventricular rotation and torsion movement,and research to twist and left ventricular ejection fraction, left ventricularend-diastolic volume,left ventricular end-systolic volume and left ventriclethree-dimensional displacement correlation.Methods:End-stage chronic kidney disease,32cases of uremicpatients, age-matched control group of30patients. Transthoracicacquisition left ventricular apical four-chamber view full volumethree-dimensional dynamic image storage, Applications onboard the3DTanalysis software for offline analysis, measurement of the peak of leftventricle of each segment and the global rotation (Prot-base, Prot-mid,Prot-ap,Prot-glo), the peak of left ventricle of each segment and the globaltwist(Ptwi-base, Ptwi-mid, Ptwi-ap, Ptwi-glo),Left ventricular segmentaland global twist angle peak time(Time Ptwi-base, Time Ptwi-mid,TimePtwi-ap,Time Ptwi-glo),left ventricular end-diastolic volume(LVEDV),left ventricular end-systolic volume(LVESV),left ventricular ejectionfraction(LVEF)and three-dimensional displacement(P3DD). Compareuremic patients with normal control group, the difference between theabove indicators, and analysis the parameters correlation of left ventricularglobal twist angle peak,left ventricular ejection fraction, left ventricular end-diastolic volume, left ventricular end-systolic volume andthree-dimensional displacement.Result: Uremic patients and normal control group, left ventricularrotation and twist curve shape and out of shape trend is consistent.Compared with the normal group, the uremia group of the Prot-mid, theProt-ap, the Prot-glo, the Ptwi-ap was significantly lower, Difference wasstatistically significant (P <0.05);The uremia group of the Prot-base, thePtwi-base and the Ptwi-mid was slightly increased, difference was notstatistically significant (P>0.05); Uremic patients left ventricular twistangle peak with LVEDV and LVESV was a negative correlation, and thatwas positively correlated with LVEF.Conclusion: The uremia group of patients compared with the normalgroup, the whole left ventricular, middle section, apical segments ofrotation and left ventricular apical segment twist angle lower, Researchshows that three-dimensional speckle tracking technology can accuratelyassess the uremic patients with left ventricular rotation and twist motionabnormalities, and to evaluate left ventricular systolic function. |
