Study On Systematized Post Training For Head Nurse And Its Effects

Objective To investigate the needs of head nurses’post training; To prepare head nurses training course by synthetically using literature searching and consulting expert; To find out the status and influencing factors of head nurses’self-efficacy, supportive interpersonal communication skills, critical thinking ability, transformational leadership styles and leadership effectiveness; To explore the application effect of systematic post training in head nurses’post training.Methods This study was a Eexperimental study.In the first stage, head nurses post training course was worked out by synthetically using literature searching, expert consulting. In the second stage, using stratified random sampling method according to the hospital grade, Recruiting80head nurses in26hospitals in Changsha area.The head nurses were divided into two groups, the experimental group and the control group randomly. There are37subjects in each group after subtracting head nurses who did not finish the experiment completely. Subjects in the experimental group were given post training for6months including60class hour face-to-face training during2weeks and Network Learning lasting10weeks and Internet exchange platform lasting3months. All subjects in both experimental group and control group were tested by Self-efficacy Scale, Supportive Interpersonal Communication Skill Scale, Critical Thinking Ability Scale, Transformational Leadership Behaviour Scale and Leadership Effectiveness Questionnaire to evaluate their abilities and Behaviours before the head nurses post training. Meanwhile, the experimental group were also required to finish corresponding index evaluation forms in different times recommended by the Kirkpatrick assessment model.Results1. Medical experts showed positive attitudes toward head nurses post training.2. The score of Counseling and Tutoring Dimention in head nurses’ Supportive Interpersonal Communication Skills was low; head nurses’Critual Thinking Ability have positive traits except for Finding Truthes Dimension;the score of Vision Incentive Dimension in Transformational Leadership was low; the score of Extra Effort in Leadership Effectiveness was low.3. The results of one-way ANOVE showed that:Among all of the head nurses’general condition, the frequency of using electronic communication ways (P=0.001) was significantly related to Supportive Interpersonal Communication Ability.the basic education degree (P=0.036), nursing ages (P=0.016) and management willing (P=0.040) were significantly related to Critical Thinking Ability.4. Multiple linear regression analysis showed:self-efficacy and the frequency of using electronic communication ways were significantly related to Supportive Interpersonal Communication Ability. Self-efficacy, nursing years, basic education degree were significantly related to Critical Thinking Ability.5. The reaction level effects of Systematic post training were good,100%of the head nurses prefer to attend this kind of training again.6. The learning level effects of Systematic post training:Paired-samples t-test showed that the score of self-efficacy, supportive interpersonal communication and critical thinking ability in experimental group were increased significantly (P<0.05),while in the control group the score were increased also but have no statistics significance (P>0.05).Two independent-samplesT test showed that self-efficacy scores after intervention between the two groups was significance (P=0.000), the scores in supportive interpersonal communication ability in experimental group were significant higher than control after intervention except for the dimension of Providing Effective Negative Feedback, the scores in critical thinking ability in experimental group were significant higher than control after intervention except for the dimensions of Seeking Truthes, Curiosity, Systematic Capacity, Cognitive Maturity. Repeated measures analysis of variance showed that the main effects of intervention on supportive interpersonal communication ability was significant (P<0.05) except for the dimension of Providing Effective Negative Feedback.There were significant interactions between time and intervention on supportive interpersonal communication ability and supportive communication dimention.(P<0.05). The main effects of intervention on the dimensions of Analysis Ability and Thirst for knowledge in Critical thinking ability was significant (P<0.05).There were significant interactions between time and intervention on Analysis Ability (P<0.05)7. The behave level effects of Systematic post training:Paired-samples t-test showed that the score of Vision Incentive Dimension in Transformational Leadership Behavior in experimental group were increased significantly (P<0.05),while in the control group the score changes have no statistics significance (P>0.05). Two independent-samplesT test showed that the score of Vision Incentive Dimension in Transformational Leadership Behavior after intervention between the two groups was significance (P=0.002).8. The result level effects of Systematic post training:Paired-samples t-test showed that the scores of leadership effectiveness in experimental group were increased significantly (P<0.05) except for the dimension of Satisfaction to the Leadership while the score changes have no statistics significance (P>0.05) in the control group. Two independent-samplesT test showed that the score of Leader Effectiveness after intervention between the two groups was significance (P<0.05)except for the dimension of Satisfaction to the Leadership.Conclusion1. Medical experts and nurse hold the view that it was necessary to carry out nurse training.2. Systematic post training could improve the head nurses’ self-efficacysignificantly.3. Systematic post training could improve the supporting interpersonal communication ability of head nurses in Counseling and Tutoring and Supporting Communication dimentions significantly.4. Systematic post training could improve the critical thinking ability of head nurses in Analytical skills and Curiosity dimentions significantly.5. Systematic post training could improvethe head nurses’transformational leadership behavior in Vision Incentive dimension significantly.6. Systematic post training could improve the head nurses’leader effectiveness in Extra effort and Leadership effectiveness dimensions significantly.
