New Method In Preventing Lymphocyst After Pelvic Lymphadenectomy

Objective:Lymphocele formation following pelvic lymphadenectomy is a frequent complication. Although there has been several methods in preventing or treating lymphocyst, but none of them got the perfect desired effect. We find a new method-scatter the talcum powder to the broken end of lymphatic vessels, and then use the remaining broad ligament to occlude the broken end-to prevent lymphocyst. The aim of the study was to discussion whether this method could be effectively in preventing lymphocyst.Methods:We had a follow-up study of104patients with gynecological malignant for3months who underwent pelvic lymphadenectomy. None of these patients had received preoperative chemotherapy and radiotherapy. These patient were randomly divided into two groups,48patients in the experimental group underwent the procedure:scattering the talcum powder to the broken end of lymphatic vessels, and then suturing the remaining broad ligament to the lateral surface of the pelvic bone for occluding the broken end. The control group just underwent interrupt suture of peritoneum after pelvic lymphadenectomy. The complications, clinical symptom and pelvic ultrasound results of both groups were contrasted to find whether the new method was effective.Results:12patients in the experimental group have lymphocyst formation after operation, the incidence is25.0%. The diameter of lymphocyst in the experimental group is (3.5±1.2)cm in equal. In contrast,26patients in the control group have lymphocyst formation, the incidence is46.4%and the diameter of the lymphocyst is (8.2±2.3)cm in equal. We use the SPSS software to analyse the results, and the difference between the two groups has a statistical significance (p<0.05).Conclusions:The new method has an obvious efficiency in preventing lymphocyst. This method can be easily mastered and the complications followed this procedure has not increased contrast to the traditional operative procedures.
