After laparoscopic lymph node dissection,the normal lymph circulation in the pelvic cavity is destroyed,and the lymph fluid in the damaged area is trapped in the pelvic cavity.The damaged lymph circulation has not been repaired and the collateral circulation has been reestablished.The lymph flow is blocked and the lymph leakage is caused,and the accumulation of the lymphocyst is formed.There are no obvious symptoms of a small lymphocele,a large lymphocyst that can pressure the ureter,the blood vessels,the lymphatic,and the bowel,which further causes the ureter's obstructive kidney fluid,lower leg vein thrombosis,lower leg edema,and bowel obstruction.Octreotide is a synthetic long-acting,natural somatostatin analogue.Relevant scholars study found that the peptide by reducing the generation of lymph,promote the healing of postoperative lymphatic leaks,reduce the number of lymphatic fluid leakage,further reduce the related complications.Relevant scholars have found that octreotide has been used in breast cancer surgery,postoperative renal transplantation,postoperative prostate cancer and so on,but it has not yet been reported that octreotide may affect the lymphatic leakage and lymphocyst of gynecological.ObjectiveTo observe the effect of octreotide on lymphatic infiltration and lymphocyst formation in female pelvic malignant tumor.MethodsFrom July 2016 to June 2017,60 patients with female pelvic malignancies of gynecological women in the affiliated hospital of Qingdao university were selected as the study subjects,and 30 patients were randomly divided into two groups.After the operation of octreotide group,Octreotide was injected subcutaneously to octreotide until the drainage tube was removed.The control group was not treated with octreotide.Record two groups of operation condition(operation time,intraoperativeblood loss,cleaning the number of lymph nodes and the number of positive,postoperative exhaust time,postoperative daily traffic,drainage time,and observe the lymphatic cyst formation and adverse drug reactions.Results1.There were 60 cases of cervical cancer and intrauterine cancer patients randomly divided into the orotripeptide group and the control group.The pathological features and general clinical data of the two groups were not statistically significant(P >0.05).2.The octreotide group and control group of operation time,intraoperative blood loss,cleaning the number of lymph nodes and the number of positive,postoperative exhaust time comparison,there were no statistically significant difference.3.The daily flow rate and drainage time of octreotide group were lower than that of the control group(96.88±28.03 vs127.84 ± 54.53,4.37 ±1.30 vs6.00±2.29),and the difference was statistically significant(P < 0.05).The octreotide group formed lymphocyst 2 cases(6.67%),all unilateral,no fever,oppression and other symptoms.In the control group,there were 8 cases(26.7%),5 cases of unilateral side,3 cases on both sides,3 cases of fever and abdominal pain,which caused the symptoms of compression,and caused swelling of the lower extremities.Among them,1 case of lymph cyst compressed blood vessels,which affected the blood circulation of lower extremities,and formed venous thrombosis.The incidence of lymphocyst was statistically different from that of the two groups(P < 0.05).4.During the administration of the octreotide group,there was no obvious adverse reaction associated with the drug,and the main adverse reactions included pain in the injection site and gastrointestinal distension,anorexia,etc.Postoperative blood sugar,blood lipid and liver and kidney work were monitored,and no obvious abnormality was found.ConclusionThe application of octreotide after laparoscopic lymph node dissection in female pelvic malignant tumor.It helps to reduce the rate of lymph cysts,reduce the formation rate of lymphocysts,and then remove the fluid from the abdomen,and then reduce the number of complications in the form of lymph cysts,and reduce the psychological burden of the patient,improve the quality of the patient,and improvethe patient's recovery and subsequent treatment.
