The Significance Of Pelvic Lymphadenectomy In Stage Ⅰ Endometrial Carcinoma

Objiective: To investigate whether pelvic lymphadenectomy is necessary for stageⅠ endometrial carcinoma.Methods: 1.A retrospective analysis was performed on the clinical data of 228 patients with endometrial cancer who underwent pelvic lymph node resection from July 2010 to September 2019 in our hospital.All patients underwent preoperative clinical staging and postoperative pathological staging.2.The lymph node spread rates of different stages of endometrial carcinoma were compared,and comparedⅠ period different muscular infiltration depth lymph node diffusivity endometrial carcinoma,and The risk factors of lymph node metastasis were analyzed by comparing the rate of lymph node proliferation in endometrial carcinoma.Results: 1.Among 228 cases of endometrial carcinoma of different stages,Lymph node metastasis was observed in 17 cases,and the metastasis rate was 7.5%,with 3.7%,7.1%,72.7% for stage Ⅰ,stage Ⅱ and stage Ⅲ respectively.2.Ⅰ stage of endometrial carcinoma with muscular infiltration,pelvic lymph node diffusion rate increased(P<0.05).There was no lymph node metastasis in patients without myoinvasion.The lymph node metastasis rate of the patients with different clinicopathological factors was also different.The lymph node metastasis rate of poorly differentiated cancer was higher than that of highly differentiated cancer;the lymph node spread rate of serous carcinoma was 100%,however,there was no significant difference in lymph node metastasis rate between the special type and adenocarcinoma.Conclusion: Ⅰ a period of patients with lymph node diffusion rate is extremely low,therefore Pelvic lymph node resection may not be considered.Ⅰ b and terminally patients should be underwent resection of pelvic lymph node.
