Reserarch Objective and background:Gestational diabetes Mellitus (GDM) is a clinical syndrome that varying degrees of impaired glucose tolerance first found in pregnancy women.It is the clinical syndrome that is caused by more than one abnormal gene expression and other environmental factors.The global incidence rate of GDM is1%-14%, which is one of the reasons that some women suffering from overt diabetes in the future. The classic theory is that the required amount of sugar and fetus during pregnancy for the amount of glucose increases and insulin secretion increased, resulting in hyperinsulinemia, to the middle and late pregnancy tumor necrosis factor (TNF-a), adiponectin (APN), leptin,increased secretion of placental hormones, insulin resistance (IR) to enhance tissue sensitivity to insulin decreased.Increased insulin secretion compensatory increase in insulin resistance caused by impaired glucose tolerance or the incidence of diabetes. Therefore, the etiology of GDM is mainly focused on the lack of insulin secretion and IR at the begine. Recent study found the addition to the above factors:the occurrence of GDM also autoimmune, genetic factors, chronic inflammation, adipocytokines, diet and exercise and other factors. Immune injury in the incidence of diabetes in which the role of the development process more and more attracted much attention. In this study, patients with GDM mother and child immunoglobulin (Ig), complement (C), T lymphocyte subsets, as well as the level of NK cells to explore the impact of GDM on the humoral and cellular immunity in the mother of.infants.Methods:1Sample collection:We chose pregnant women who can diagnose of GDM, diet to control blood sugar ideal patients (GDM1group)30cases; diagnosis of GDM controlled by dietblood sugar is not ideal, the need to increase the patients treated with insulin (GDM2group)28cases in Shandong Provincial Hospital for the study; and normal healthy pregnant women (healthy controls),30cases as controls. Patients with GDM and normal pregnant women take to be produced in the early morning, fasting venous blood samples (fasting≥8hours); neonates immediately after the umbilical take the umbilical cord blood, the serum was separated within24hours after the test2The moumt of CD3+, CD4+, CD8+T cell and NK cell was mesured by flow cytometry.3. The mount of IgG、IgA、1gM、C3+、C4+were measuredby immunoasay4. Analyses were performed using the statistical software package SPSS17.0.Results:1. peripheral blood T lymphocyte subsets and NK cells compared with healthy control group compared two groups of patients with GDM CD4+and CD4+/CD8+, CD16+CD56+content decreased, of CD8+content increased, the difference was statistically significant (P<0.05); of CD3+content decreased, with no significant difference (P>0.05).2. peripheral blood immunoglobulin and complement levels with the healthy control group of GDM in peripheral blood of patients with the C3+, C4+, IgE levels increased IgG content decreased, the difference was statistically significant (P<0.05); IgA, and IgM content decline, with no significant difference (P>0.05).3. compared with the healthy control group of GDM patients with umbilical cord blood in CD3+, CD4+, CD4+/CD8+, CD16+CD56+content were decreased, the difference there is significant (P<0.05); of CD8+content increased, with no significant difference (P>0.05).4, compared with the healthy control group, GDM patients with cord blood IgM and IgG content of decline, of IgE levels increased, but the difference was not statistically significant (P>0.05).Conclusion:1GDM Mother and their children are have cellular immunity、humoral immune dysfunction,so they are easy to induce infection. GDM.2, Compared with normal pregnant women and their children, gestational diabetes mother and children’s CD4+counts decreased, CD4+/CD8+ratio decreased NK values decreased, indicating that immune suppression weakens an immune imbalance.3, Compared with normal pregnant women, pregnancy of the diet control group of diabetic patients of IgA, IgE, IgM and IgE levels without significant changes in insulin treatment group, patients with GDM IgE was significantly increased and IgG decreased. This shows that patients with GDM there are humoral immune disorders and more severe disease, functional damage is more obvious.4, the study also found that patients with gestational diabetes C3+, C4+, are elevated to C3+increased significantly.5, the results of this study show that immune function in GDM birth compromised and impaired cellular immunity-based...
