Bacground Earthquake is one of the worst natural disaster, whichusually gives human destructive coup. Afer the earthquake or large scaledisaster happen, the international medical scientific research institutionsalways invest a large of people, material and money on study ofepidemiology, locus patient rescue, clinical treat and coping mode in thefuture.Most of rusults have been reported in articls by reseachers ofdeveloped country and these related data is dispersed in individualsinstitution or researchers. Until now, China is lack of earthquake traumadatabase. A project conducted by West China Hospital of SichuanUniversity is supported by National High Technology Research andDevelopment Program of Chin(a863)and the mainly content is collectionand analyze the clinical characteristics of various injury, establish5.12earthquake trauma database and summarize remedy experience. Amongthem, screening and confirm the earthquake trauma information collectionindex is the important content.Aim Screening and confirm the index system of5.12earthquaketrauma database according to availability and utilization. Evaluate thereliability and validity of the index using preliminary experiment. At last,analyze characteristics of earthquake trauma.Method First, establish frame structure and index system of scheduletable by means of systematic analyzing literature related to earthquakeinjury and summarizing the index system correlation with eathquake injuryanalysis.Second, consummate the index by expert consulation three times. Third, evaluate the reliability and validity of the index using preliminaryexperiment twice.Result (1)Frame structure of index system::According to the principleof earthquake trauma patient rescue by stages, the information collectionincludes eight parts, i.e. general information, locus in quo information,transmit and transportation information, emergency information,hospitalization traumatic condition, specific treatment information,rehabilitation care information and discharge information.(2)Establishindex system: According to the results of systematic analyzing literature,three times expert consulation and twice preliminary experiment, the indexsystem includes336items screening from369items.(3)Consulation expertmainly come from lage hospital or university specializing in brain trauma,bone injury, chest trauma, abdomen trauma, occlusalf surface trauma,multiple injury, hemorrhagic shock, crushing syndrome, ARDS, MODS,severity infection, gravida injury,children injury and old age injury,epidemiology and evidence based medicine.(4)Analyze characteristics ofearthquake trauma: Traumatic condition information of51746in hospitalearthquake injury patients were collected and we analyzed the gender ratio,age distribution, injury type, injury position, injury reason andcharacteristics of various earthquake injuries. The rusults showed that theaverage stay of earthquake injury patient was18.8days, medium stay was9days and a small admission peak was emerged6to8days afterearthquake. Male patients occupied51%of total. The average age of thesepatients was45years old. The average time from injury to first aided was16.8hours and medium time was3hours. The average time from injurytofirst treated was29.5hours and the medium time is12hours. The mainlyinjury reseaons were crush injury(68.0%), fall down/crash (17.1%)) andcrushing/bury(7.9%). Limbs(46.9%), cephalosome(14.7%) and chest(9.0%) were the first three frequent positions in earthquake injury patients. Closedinjury occupied51.5%of the total, while open injury occupied48.5%. Theaverage crushing/bury time was20.6hours and the medium time was6hours. Only8.3%wounded people were remedied locus in quo among8809recorded patients. Among all the in hospital injury patients,32.5%patients accepted operation,11.6%patients accepted blood transfusion and1.9%occurred shock, crush syndrome and/or MODs.In all,1307patientsdied in hospital among37516recorded patients and the case fatality was3.5%.Conclusion Natural disaster such as earthquake will frequently takeplace in the future human history and we can’t forecast them exactly,especially earthquake. The advanced analysis of the earthquake databaseinformation is help to summarize the medical experience and lession ofWenchuan earthquake and eatablish earthquake trauma emergency remedyschedule.
