Objective:To observation and analysis the clinical efficacy and its correlation withradiographic changes with radiofrequency thermocoagulation through the inside facetof the small joint treatment of the single-gap lateral lumbar disc herniation.Methods:With the strict standard,50patients with LDH (the single-gap lateral lumbar discherniation)who were therapy in the First Affiliated Hospital of Nanchang Universitypain inpatient from March2009to March2011included in the research, and in all50discs operated with the radiofrequency thermocoagulation through the inside facet ofthe small joint. All patients included in the research are performed by the samephysician with high qualification. At the pre-surgery、four days after、three monthafter、twelve month after, four time points using the pain visual analogue scale (VAS)as the pain level evaluation; using Japanese Orthopaedic Association (JOA) toassessed the low back pain disorders efficacy criteria; three month and twelve monthafter the treatment, review of lumbar intervertebral disc CT,measurements the indexof vertebral disc herniation (IDH), the disc height (DH) in the PACS system software,and calculate the change before and after to analysis the correlation between the JOAscore and the rate of imaging change. SPSS statistical software analysis before andafter treatment changes, correlation and impact analysis of all relevant factors, P<0.05was considered statistically significant.Results:(1)、No serious complications in all patients after treatment.(2)、With an averagelength of stay (9.56±1.227) d;(3)、Before surgery, the VAS (4.60±0.78); four daysafter、three month after、twelve month after, the VAS scores were (1.46±0.78),(2.2±0.64)(2.26±0.63); preoperative and postoperative VAS scoresthere weresignificant differences; four days after、three month after、twelve month aftertreatment, JOA excellent and good rate was72.5%,82%,86%, the12monthspostoperative is finer than the four days after、three month after;(4)、pre-surgery、 four days after、three month after、twelve month after treatment the IDH is:(0.28±0.03)(0.25±0.02)(0.21±0.02); the DH is:(7.72±0.35) cm (7.21±0.33)cm,(6.17±0.29) cm; three month after、twelve month after treatment, the clinical efficacy andradiographic changes compared with preoperative significant difference (P<0.05);(5)、to analysis the correlation relationship between the JOA score and theindex of vertebral disc herniation、disc height in three month after、twelve monthafter treatment,the results suggest that postoperative of3months and12months JOAscore with the index of vertebral disc herniation were significant correlated.Conclusion:(1)、Lumbar disc radiofrequency thermocoagulation for lumbar disc herniation issafe and effective;(2)、The clinical efficacy of radiofrequency thermocoagulationtherapy of lumbar disc herniation is stability,long-term、 short-term effects isundifferentiated;(3)、The clinical effect and the narrow of protrusion、intervertebralheight decreased have a certain relationship, and the protrusion narrow than the discspace height reduction more meaningful clinical effect.
