Objective:To observe the effects in anatomy of the positioning manipulation in the lateral type of atlantoaxial joint malposition, to discuss the mechanism of the positioning manipulation in the lateral type of atlantoaxial joint malposition,Methods:a randomized controlled trial of research methods, the clinical data of30cases with diagnostic criteria, inclusion criteria for the case, using standard control method, the collected cases in accordance with the complete randomization principle and divided into experiment group (group A) and control group (group B), the test group by atlantoaxial joint positioning manipulation method, control group using traction method as the processing factors, observation of spacing difference in odontoid process to Lateral mass of atlas, the spacing difference in lateral atlantoaxial joints, the spacing difference in axis spinous process deviation before and after the treatment, and observation of clinical symptoms improvement. The test results using SPSS11statistic analysis software for data processing, and do the statistics and analysis to the test data.Results:in group A,before and after treatment the spacing difference in odontoid process to Lateral mass of atlas was significantlly different (P<0.05), group B before and after the treatment the spacing difference in odontoid process to Lateral mass of atlas was not significantlly different (P>0.05); in group A before and after treatment the spacing difference in lateral atlantoaxial joints was not significantlly different (P>0.05), in group B before and after the treatment the spacing difference in lateral atlantoaxial joints was not significantlly different (P>0.05), in group A before and after treatment the spacing difference in axis spinous process deviation was significantlly different (P<0.05), in group B before and after treatment the spacing difference in axis spinous process deviation was not significantlly different (P>0.05), before and after tretment clinical symptom improvement was significantlly different in A and B.Conclusion:in the lateral type of atlantoaxial joint malposition, the positioning manipulation can improve the position relationship between the odontoid process and lateral mass, and traction can not improve the position relationship between the odontoid process and lateral mass, the positioning manipulation and traction can not improve the position relationship in lateral atlantoaxial joints;the positioning manipulation can improve the relative location between the spinous process and the odontoid process, but this was not significant significant improvement in traction. Improvement in clinical symptoms, the positioning manipulation was obviously superior to the traction treatment.
