Objective:To observe the clinical effect of xiang-fu-si-wu decoction as a treatment for primary dysmenorrhea (qi stagnation and blood stasis syndrome), and its impact on the level of E2and P.Methods:60patients with primary dysmenorrhea due to qi stagnation and blood stasis were randomly divided into a treatment group (30patients) and a control group (30patients). The two groups were seperately treated with xiang-fu-si-wu decoction and yue-yue-shu particles both for three cycles, and then observed for three cycles. At the same time every patient in the treament groupe were drawed blood three times while be treated at the first12hours of every cycle, while the first blood drawing must be before having drugs. Pain and TCM syndrome scores were observed before treatment, and after3and6cycles of treatment,as well as the changes of level of E2and P..Results:1.At the effect of relieving pain, the scores of the3rd and6th cycle of both groups were significantly lower than before treatment (P≤0.05), saying that both groups can significantly relieving pain;the difference between the3rd and6th cycle in the treatment group was not remarkable(P>0.05),while in the control group was significant(P≤0.05), saying that the durance in the treatment group was remarkably better than the control group. There were no significant differences in both the scores of the3rd and the6th cycles between the two groups(P>0.05),saying that the effect of relieving pain of the two groups were similar.2.At the effect of relieving TCM syndrome, the scores of the3rd and6th cycles of both groups were significantly lower than before treatment (P≤0.05),saying that both groups can significantly relieving TCM syndrome; The total effect rate of the3rd and6th cycle in the treatment group were seperately76.7%and73.3%,while in the control group were seperately80%and83.3%.The differences of the3rd cycles rates between the two groups and the6th cycles rates between the two groups were both not significant (P>0.05), saying that the effects of relieving TCM syndrome in both groups were similar. There were no relations both between the pain scales and the effect of the medicine and between the disease durance and the effect.3.At the impact on E2and P andE2/P,compared to before treament, there were great changes on the level of E2in the second cycle(P≤0.05), saying that the level of E2in the second cycle was lower. P and E2/P in the second cycle were no significant changes compared to before treatment (P>0.05).On3rd cycle, P and E2/P in the3rd cycle were both significantly different compared to before treatment(P≤0.05). saying that the level of P in the3rd cycle was significantly higher, while the level of E2/P in the3rd cycle was significantly lower compared to before treatment. At the same time there were no significant differences on the level of E2in the3rd cycle compared to before treatment.(P>0.05).Saying that the level of E2in the3rd cycle was no changes, compared to before treatment. Conclusion:The xiang-fu-si-wu decoction therapy for qi stagnation and blood stasis syndrome of dysmenorrhea exhibits a secure efficiancy of pain and TCM syndrome relieving, and indicates that the efficiancy comes true by regulating the level of E2and P.
