Clinical Experience Of Professor Ll Li-yun In Treatment Of Infertility

Object ivesTo summary the diagnosis and treatment of infertility of Professor Li Liyun further in-depth on the basis of previous clinical experience and clinics of Professor Li infertility treatment experience summary and clinical drug studies and to make tacit knowledge explicit, improve the clinical experience heritage efficiency.MethodsThis was a retrospective study. First, collect the information from Professor Li Liyun outpatient treatment of infertility cases of female patients from January,2011to December,2011and fill in the cases observed table. The main observations were including history, menstrual history, physical examination, laboratory examinations, diagnosis, treatment and medication (including the TCM prescriptions, Western medicine prescriptions, traditional therapy, etc.). Second, establish a database, using frequency analysis, chi-square test, non-conditional logistic regression analysis, analyze the clinical experience and academic thought of Professor Li in diagnosis and treatment of infertility.Result1. Internal treatment of infertility(1) Internal treatment of different stages of the menstrual cycleClinic information included in this study of409cases after sorting by treatment and different stages of the menstrual cycle showed that the treatments in the menstrual period were the most commonly used enriching the kindey and activating blood, followed by warming the kindey and invigorating yang, nourishing the kindey yin; in the follicular phase were the most commonly used warming the kindey and invigorating yang, followed by enriching the kindey and activating blood, nourishing the kindey yin; in the ovulation period were the most commonly used enriching the kindey and freeing the collateral vessels, followed by warming the kindey and invigorating yang; in the luteal phase were the most commonly used warming the kindey and invigorating yang.(2) Internal treatment of kidney deficiency infertilityIn Professor Li Liyun clinical treatment of various types of infertility, warming the kindey and invigorating yang (35.7%) was the most commonly used, followed by enriching the kindey and activating blood (15.2%), nourishing the kindey yin (11.7%), enriching the kindey and freeing the collateral vessels (11.2%).For patients with kidney deficiency infertility, warming the kindey and invigorating yang was related with age, blood clots, stage of the menstrual cycle, basal body temperature (P<0.05), which the basal body temperature in the high-temperature phase by this treatment was21.6times of the low-temperature phase (95%CI:2.509-185.923). Enriching the kindey and activating blood was related with menstrual flow, menstrual color, stage of the menstrual cycle (P<0.05), which menstrual late by this treatment was17.4times of the normal ones (95%CI:2.509-185.923). Nourishing the kindey yin was related with stage of the menstrual cycle, light red tongue, red tongue, thin yellow tongue coating (P<0.05), which in the luteal phase by this treatment was0.2times of ones not in this phase (95%CI:0.037-0.849). Enriching the kindey and freeing the collateral vessels was related with blood clot, stage of the menstrual cycle, B ultrasound showed the advantages/mature follicle, thin pulse, taut pulse (P<0.05), which with premenstrual breast tenderness by this treatment was15.8times of wi thout breast tenderness (95%C1:2.824-87.952), in the ovulation period by this treatment was8.5times of ones not in this period (95%CI:4.301-16.657), i n the luteal phase by this treatment was3.2times of ones not in this phase (95%C1:1.682-6.190).2. External treatment of infertilityAmong the external treatments of Professor Li Liyun treating infertility patients, traditional Chinese medicine package and Si-huang powder were used in the highest rates (18.6%,5.6%). Use of traditional Chinese medicine package was related with uterine position, annex inflammation, tubal patency check (P<0.05), which adnexitis in use of traditional Chinese medicine package was4.8times of ones non-Annex inflammation (95%CI:1.408-16.425), tubal patency abnormalities in the use of traditional Chinese medicine package was5.7times of non-abnormalities ones (95%CI:2.245-14.563). Use of Si-huang powder was related with cervicitis, uterine position, adnexitis (P<0.05), which adnexitis in use of Si-huang powder was4.8times of non-adnexitis ones (95%CI:1.114-20.531).3. Treatment of infertility by integrative medicineProfessor Li clinical treatment of ovarian dysfunction infertility, the most common used treatment was a single internal medicine modulating (43.4%), followed by aiding with ovulation drugs (20.0%). The most common used treatment of tubal infertility was internal Chinese medicine with topical Chinese medicine (49.0%), followed by a single internal medicine (33.7%). The most common used treatment of other types of infertility was a single internal medicine (55.6%).ConelusionIn clinical diagnosis and treatment of infertility, Professor Li Liyun stressed on combining disease and syndrome, defense to kindey-based. Chinese medicine conditioning treatment was according to the analysis of different stages of the menstrual cycle, and flexible use of warming the kindey and invigorating yang, nourishing the kindey yin, enriching the kindey and freeing the collateral vessels, enriching the kindey and activating blood, timely tune up the yin and yang. On this basis, Professor Li integrated multi-channel administration, such as ovarian dysfunction supplemented with western medicine ovulation, tubal infertility supplemented with external Chinese medicine to improve the pelvic microenvironment.
