BackgroundCervical spondylopathy of cervical type is the early stage of invasion on various types of cervical spondylopathy. It is commonly seen in clinic and easy to suffer. The majority are middle-aged and elders.The incidence rate of cervical spondylopathy was significantly higher than before because of the development of society, the increase of working strength and workload, the popularization and application of computers and cars, the augment of student’s homework pressure and so on. And the trend of incidence age of these patients gets younger and younger.In the next50years, the disease is becoming a kind of modern disease accompanied with modern society.At present, manipulation treatment is an important part in none-surgical therapies for cervical spondylopathy. It’s efficiency has been proved by clinical practice and experimental research.There are certain risks and limitations for the previous manipulation therapy such as large range of motion, diffcult to grasp the intensity of manipulation, easy to lead to new damages to cervical vertebra and surrounding soft tissue, focus on local effect and whole effect which affect and limit the curative effect and safety of manipulation therapy. In the middle of last century, Liyun Liu, a professor of college of sports medicine clinic in Haerbin institute of sport, founded proprioceptive sensibility reflexotherapy through28years’ research, which was based on holistic concept of TCM, athletic physiology and sports medicine. It treats disease by using manipulation to stimulate the stretch reflex organ and by using the proprioceptive reflex to promote, regulate and improve local body and internal emironment of human body.So, it can provide a new concepts and ideas for the treatment of spinal source disease and soft tissue injury by manipulation therapy.So far, there is no clinical efficacy studys about various types of cervical spondylopathy treated with proprioceptive sensibility reflexotherapy. This project is a rolling one under study of action,using proprioceptive sensibility reflexotherapy to treat cervical spondylopathy of every type by my tutor recent yearsObjectives1. To investigate the clinical effect on cervical spondylopathy of cervical type treated with proprioceptive sensibility reflexotherapy by clinical randomized controlled study.2. To observe if the curative efficacy of proprioceptive sensibility reflexotherapy is superior or equivalent to maitland manipulation3. To provide theoretical basis for technical specification and clinical exchanging popularization of cervical spondylopathy of cervical type treated with proprioceptive sensibility reflexotherapyMethods1. Subjects:42patients with cervical spondylopathy of cervical type who met the inclusion criteria from the Dept of Rehabilitation of Guangdong Provincial Hospital of TCM were recruited into this study from December2010to February2012.2. Randomization:All subjects were randomly assigned to two groups by using random number method, the treatment group21subjects and the control group21subjects.3. Intervention:Treatment group: Proprioceptive sensibility reflexotherapy+Packet treatment of traditional Chinese medicine+Computer modulation medium frequency electrotherapy; Control group:Maitland manipulation+Packet treatment of traditional Chinese medicine+Computer modulation medium electrotherapy. A course is6times treatments.4. Outcome measurements:visual analog score (VAS), pressure pain measurement was measured (kg/cm2and six directions range of motion of cervical vertebra before the1st treatment and after the2nd/4th/6th treatment.5Statistics:Analysis was performed using the statistical package of social science (SPSS),version18.0.Independent sample t test was used to compase the difference before and after treatment between the two groups. Paired samples t test was used to compase the difference before and after treatment either treatment group or control group. Statistical significance was set at the5%level. Result1. The general information of two groups includes gender, age, course of disease. No statistically significant difference were found in the general information and visual analog score (VAS), the pain threshold and tolerance threshold of pressure pain points (kg/cm2), cervical six directions range of motion before the treatment (P>0.05). Indicating that the baselines of two groups were comparable.2. The visual analogue scores, pain threshold and tolerance threshold, range of motion of cervical vertebra were statistically significant higher than that of before treatment in two groups (P<0.05). The effective rate of the treatment group was90.47%, the control group was85.71%.3. After treatment,there was statistically significant difference (P<0.05) in the improvement rate of pain threshold and tolerance threshold between two groups.4. After the6th treatment, the average magnitude of the change of the analogue scores of Jianjing point was2.543±0.963, higher than that of the control group (P<0.05). There was no statistically significant difference in visual analogue scores between two groups (P>0.05), at other time point.5. After the2nd treatment, the average magnitude of the change of right lateroflexion’s range of motion was4.413±4.258, higher than that of the control group(P<0.05). There was no statistically significant difference in six directions range of motion of cervical vertebra between two groups (P>0.05), at other time point.6. After the all treatment, there was no local skin flushing, petechiae and other in the two groups.Conclusion1. Both proprioceptive sensibility reflexotherapy and maitland manipulation can relieve pain and enlarge the range of motion of cervical vertebra2. Proprioceptive sensibility reflexotherapy has the better effect than maitland manipulation in pain threshold and tolerance threshold.
