ObjectivesIn recent years, Chronic Obstructive Pulmonary Disease (hereinafter referred to as COPD) combined with chronic cor pulmonale (hereinafter referred to as chronic cor pulmonale) as the most common and frencely disease of the respiration system, as the pulmonary function is progressive decline, and it serious influence of workers and patients quality of life, and the disease has a high prevalence and high mortality rates, is not only a national health killer, on the other hand, also greatly stimulate the medical expenses rise sharply, bring heavy economic burden to social and family. Chinese medicine treatment and intervention measures based on the dialectical theory in the chronic disease management of COPD with chronic cor pulmonale patients should not be ignored. Traditional Chinese medicine in the clinical diagnosis and treatment plays an important role, especially the role of the syndrome of TCM in COPD combined with chronic cor pulmonale disease can not be ignored. But it is lack of uniform diagnostic criteria research of TCM syndrome, and the clinical evaluation of a certain lack of convincing.PurposeThrough to analysis the inpatient and outpatient patient’s syndrome of Chinese Medicine of COPD with chronic cor pulmonale in the Traditional Chinese Medical Hospital of Guangdong Province, to collect and analysis the symptoms, then from patients the distribution of TCM syndrome and it possible associated factors, for the future of establish syndrome of TCM of clinical standard of the COPD with chronic cor pulmonale to provide a preliminary basis.MethodsThis research was conducted strictly at Guangdong Provincial Hospital of Traditional Chinese Medicine. In accordance with the predetermined standard,100cases of patients who were diagnosed as COPD with chronic cor pulmonale detailed extraction the principal and less important clinical symptom, coating on the tongue, pulse, predisposing factors and so on. Data were collected to analyze by using Excel software for the establishment of database, and using SPSS17.0statistical software. The relationship of TCM syndromes between the related factors was also analyzed.Results1. Within patients of chronic obstructive pulmonary disease combined with chronic cor pulmonale, smoking is still ranked the first. Six exogenous pathogenic were mainly regarded as resulting in acute exacerbation of the disease, the diets factor was in the next place. The investigation of this study, COPD pulmonale function is mainly for the classification II and classification Ⅲ.2. The syndrome of phelgm stagnating the lung, the syndrom of deficiency of the kidney, lung and spleen, heat-phelgm and blood stasis stagnating the lung, syndrome of deficiency of Qi of lung and kidney, phelgm-heat stagnating the lung, blood stasis, which have high frequency are mainly syndrome patterns of the COPD with chronic cor pulmonale. The empirically syndromes in a descending sequence as follows:phelgm stagnating the lung, heat-phelgm and blood stasis stagnating the lung, phelgm-heat stagnating the lung, blood stasis, wind chill bunch of lung. And the deficiency syndromes as follows: the deficiency of the kidney, lung and spleen, the deficiency of Qi of kidney and lung, Yin deficiency of kidney and lung, deficiency of the lung and spleen, syndrome of moisture ling to heart. And this disease are more the empirically syndrome mixed up with deficiency syndromes.3. The patients with disease severity scores in CAT account for the largest proportion, but with no statistical difference between the groups of syndromes in Traditional Chinese Medicine. While the classification of pulmonary dysfunction and syndromes of Traditional Chinese Medicine have statistically significant differences in pulmonary function, patients with blood stasis syndrome are the most common in classification11, the empirically syndrom of phelgm-heat stagnating the lung, syndrome of deficiency of lung and spleen, syndrome of phelgm stagnating the lung, which have high frequency are mainly syndrome types of the pulmonary function classification Ⅲ, whereas syndrom of deficiency of the kidney, lung and spleen, syndrome of phelgm stagnating the lung, are the most common in pulmonary function classification IV.Conclusions1. COPD with chronic cor-pulmonary disease is a serious station in the development of COPD. For the patients, who with more for long-term smoking history, positive advice to give up smoking is great significance. The disease have a characteristic of long, more aggravating and repeatedly, and it still can’t cure at present.2. The disease have complexed etiology and pathogenesis, in clinical empirically syndrome always mixed up with deficiency syndromes, in acute exacerbation period, the main syndrom are phlegm stagnancy, phelgm-heat stagnancy, syndrome of blood stasis, and which can influence each other. In the more stable period, the main syndrom are the deficiency of the kidney, lung and spleen, the deficiency of Qi of kidney and lung.3. COPD with chronic cor pulmonary disease with severity of disease classification and distribution characteristics of TCM syndrome manifested by the characteristics of the pathogenesis is the initial period to more standard, later to see this standard is false card is consistent with. Pulmonary heart disease staging and CAT score between disease states are mutually consistent, pulmonary heart disease patients with decompensated for disease severity status.
