Objective:To determine clinical sificance of serum NT-proBNP and their dynamic characteristics in patients with cardiac valvereplacement combined with radiofrequency modified maze procedure(Ⅲ).To explore clinical value in predicting perioperative cardiac function and in guiding the Treatment after cardiac valvereplacement combined with radiofrequency modified maze procedure(Ⅲ).Methods:We retrospectively studied98patients undergoing cardiac valvereplacement combined with radiofrequency modified maze procedure(Ⅲ),examinations ECG,measuring LVEF and LAD use of Doppler echocardiography before surgery.Plasma NT-proBNP levels were respectively measured in patients in1day before operation and postoperative1h,1to7days by radioimmunoassay, and all patients followed up3months. To analyse the change of perioperative NT-proBNP leves, the relationship of between preoperative NT-proBNP concentrations and cardiac function (NYHA), LVEF. And the clinical value in predicting of recurrence atrial fibrillation after cardiac valve replacement combined with radiofrequency modified Maze procedure (Ⅲ).Results:All the98patients were involved in the final analysis.1.In the first determination after surgery NT-proBNP levels were similar to preoperative levels(p<0.03). However NT-proBNP levels started increasing the next day aftersurgery. NT-proBNP levels of the second, third day were still higher than the preoperatory datas. In the seventh day NT-proBNP levels were gradually decreased than preoperative levels(p<0.03).2.It may be observed postive correlation of preoperative NYHA class classification and preoperrtroy NT-proBNP levels (p<0.05).There is negative correlation of LVEF and preoperatory NT-proBNP levels (p<0.05). 3.NT-proBNP levels in the AFgroup were significantly higher than those in the SR group before and after operation (P<0.05). Plasma NT-proBNP levels decreased significantly after operation in the two group (P<0.05)Conclusion:1、There is a good relationship between pre-operative plasma NT-proBNP levels and pre-operation cardiac function2、pre-operative plasma NT-proBNP levels has a clinical value in predicting of recurrence atrial fibrillation after cardiac valve replacement combined with radiofrequency modified Maze procedure.
