The Value Of Brush Biopsy And Brush Washing Liquor To Improve The Diagnostic Sensitivity In Bronchoscopic Examination

Objective:To investigate the value of bronchoscopy combind with brush biopsy, brush washing liquor and bronehoalveolar lavage fluid to improve the diagnostic sensitivity of lung cancer.Methods:Total of294patients, include who have lesion under CT, examined by bronchosopy with brush biopsy, brush washing liquor, bronehoalveolar lavage and forceps biopsy. Put the brush washing liquor and lavage into ThinPrep Cytolyt test, all the specimens were sent to pathology office to examine.Results:128patients (43.5%of all) were confirmed diagnosed by fibrobronchoscopy observing. They includes46cases with mucous invasive change,16cases with intratrachal neoplasm,47cases with tracheal lumen stenosis,16cases with mucous inflammation,13cases with no abnormality.94patients were confirmed diagnosed by biopsy through bronchoscope,44patients were detected by brush biopsy,40patients were detected by forceps biopsy,30patients were detected by bronchoalveolar lavage. There were34negative patients using fibrobronchoscopy forceps biopsy, including25confirmed diagnostic patients by CT-guided percutaneous pulmonary biopsy and8patients by operation. There were40squamous cancer cases,44adenocarcinoma cases,22undifferentiated carcinoma cases,8small cell carcinoma cases,6genotyping carcinoma cases,8other carcinoma cases in all patients.Conclusions:Brush biopsy, brush washing liquor,lavage combine with forceps biopsy can improve the positive rate of diagnosis of lung caner. Obtain brush washing liquor after brush biopsy can improve brush biopsy’s diagnostic sensitivity of lung cancer,moreover it is safe and convenient. Adenocarcinoma is the highest proportion in the variety of pathological types in female and non-smoking patients. Squamous lung cancer obviously relate to smoking.
