Objective: To discuss the expression of CEA and VEGF in the bronchoalveolar lavage fluid and explore its application value in the diagnosis of lung cancer.Methods: CEA and VEGF level in BALF were detected of 30 patients from lung cancer group and 30 patients from lung benign disease group in Yijishan Hospital affiliated to Wannan Medical College from October 2013 to October 2014.Results:(1) The levels of CEA and VEGF in BALF and serum in patients from lung cancer group were significantly higher than the patients from lung benign disease group, the levels of CEA and VEGF in BALF were significantly higher than serum in patients from lung cancer group, the differences all have statistical significance(P < 0.05).(2) The level of CEA in adenocarcinoma was the highest in different histological types in BALF and serum of patients from lung cancer group, the differences have statistical significance(P < 0.05); while there was no statistical significance between squamous carcinoma and small cell carcinoma(P > 0.05); The level of VEGF in squamous carcinoma was higher than adenocarcinoma in BALF of patients from lung cancer group, the difference has statistical significance(P < 0.05), there was no statistical significance between squamous carcinoma and small cell carcinoma(P > 0.05), and there was no statistical significance between adenocarcinoma and small cell carcinoma(P > 0.05); The level of VEGF in serum of patients from lung cancer group has no statistical significance among different histological types(P > 0.05).(3) The level of CEA was associated with TNM stage in BALF and serum of patients from lung cancer group, the differences have statistical significance(P < 0.05); while the level of VEGF has no association with TNM stage in BALF of patients from lung cancer group(P > 0.05), the level of VEGF in stage I-II was higher than stage III or stage IV in serum of patients from lung cancer group, the differences have statistical significance(P < 0.05), there was no statistical significance between stage III and stage IV(P > 0.05).(4) The area under the ROC curve of CEA and VEGF detection alone and combined in BALF were 0.895, 0.861, 0.952, respectively, while the sensitivity were 90%, 93.3%, 90.0%, respectively, and the specificity were 86.7%, 86.7% and 93.3%, respectively, the critical point of clinical diagonosis were 7.9 ng/ml, 191.2 pg/ml.(5) There were positive correlation between the expression of CEA and VEGF in BALF of patients from lung cancer group(r = 0.458, P < 0.05), while the expression of both in serum has no obvious correlation(r=0.133,P>0.05).Conclusion: The measurement of CEA and VEGF in BALF has a certain application value in the diagnosis of lung cancer, Combined determination might be useful to improve the diagnostic level.
