The clinical value of serum tumor marker CA19-9in the diagnosis of suspected pancreatic cancer by imaging methodsBackground and objective:To explore the clinical value the combination of serum tumor marker CA19-9and imaging methods (ultrasound, computed tomography, and magnetic resonance imaging) in the diagnosis of pancreatic cancer.Methods:The clinical data of343patients with pancreatic disease, who were finally diagnosed by pathology from January,2010to February,2012were retrospectively analyzed.Results:Combined with the pathological findings of343cases, the sensitivity of B-ultrasound, CT and CA19-9of the diagnosis of pancreatic cancer are72.2%,89.2%and79.5%respectively, and the specificity of them are68.3%,54.4%and72.7%respectively. The pathological findings showed that, in all267patients who were firstly diagnosed as pancreatic cancer using imaging methods,225(83.0%) patients were diagnosed as pancreatic cancer and42(17.0%) patients non-pancreatic cancer. In the pancreatic cancer group, serum CA19-9was elevated in173(76.9%) patients, while normal in52(23.1%) patients.85patients with suspected pancreatic cancer by imaging methods had normal CA19-9value. Among them,52(61.2%) patients were finally diagnosed as pancreatic cancer on pathology, and33(38.8%) patients as non-pancreatic cancer.182patients with suspected pancreatic cancer by maging methods had elevated CA19-9value. Among them,173patients were finally diagnosed as pancreatic cancer on pathology and9patients as non-pancreatic cancer.Conclusion:Serum tumor marker CA19-9may provide valuable clues for the diagnosis of pancreatic cancer. As to CA19-9negative patients, in whom pancreatic cancer was suspected by imaging methods, much more attention should be paid to the possibility of false positive results of imaging methods. Pancreas puncture before surgery is recommended to confirm the diagnosis. As to patients with elevated CA19-9and imaging methods suspected pancreatic cancer, pancreas puncture before adjuvant therapy is also recommended.
