ObjectiveTo investigate the safety of Transpedicular Bone Grafting with7mm Bone Grafting Funnel in treatment of thoracolumbar fractures by Observing its influence on vertebral pedicle.MethodsForty seven patients undergoing posterior pedicle fixation and transpedicular bone grafting were retrospectively evaluated by analysing the CT scanning images. Compare the perforation rate of pedicles between bone grafting and pedicle screwing, the transverse and inner diameter of pedicles with grafting measured by Pacs Client software(PACS) both between pre-operation and post-operation and between perforated and non-perforated.ResultsThere were no significant differences between grafting and screwing in perforation rate(P>0.05) of pedicle lateral wall, but it was much higher in perforation rate of medial and total wall for grafting(P<0.05). The perforated pedicles were much narrower than non-perforated ones either pre-operation or post-operation(P<0.05), and all the pedicles were enlarged after bone grafting(P<0.05).Conclusion7mm bone grafting funnel may increase the perforation risk of pedicle for Chinese thoracolumbar fractures, so improvement should be needed for safer transpedicular bone grafting. However, measuring the transverse and inner diameter of pedicles of fractural segment before operation and choosing a suitable pedicle to graft may be useful to reduce perforation rate. Of course, welldesigned plan and careful operating should be necessary too.
