Objective To investigate the clinical effectiveness of pedicle screw fixation and the transpedicle bone grafting treatment of thoracolumbar fractures. Methods We retrospected 21 cases of thoracolumbar fractures with the pedicle screw fixation and transpedicle bone grafting treatment, and compared to 26 cases with pedicle screw fixation and bone grafting between the transverse process treatment by radiologic outcomes measured after operation and during the follow - up period including the changes of the anterior body height and the loss of the Cobb's angle correction. Results There were no significant differences between the 2 groups in either the anterior body height or Cobb angle correction in immediate postoperation. But in postoperative one-year follow-up, both the anterior body height and Cobb angle correction had no significant changes in pedicle screw fixation and the transpedicle bone grafting treatment group, and changed obviously in pedicle screw fixation and bone grafting between the transverse process treatment group.Conclusion Pedicle screw fixation and transpedicle bone grafting treatment of thoracolumbar fractures is an effective way to maintain the anterior body height of vertebra and reduce fixation failure.
