Analysis Of Adolescent Vitreoretinal Surgery

Objective:Research and analyze the clinical features of vitreoretinal operation for teenagers, give an overall evaluation of factors related to the operation and offer guidance to the follow-up clinical work.Method:Collect47cases of vitreoretinal diseases and48eyes with teenager patients as18years old or younger than18from January2006to December2010. Do statistical analysis for their related clinical data. According to different pathogeny, divide them into two groups. One is primary vitreoretinal diseases with25cases and26eyes, and the other is traumatic vitreoretinal diseases with22cases and22eyes. Discuss and analyze their aetiological agent, operation method, related disposal during the operation and prognosis.Result:Among all cases, the main reason of vitreoretinal diseases is traumatic vitreoretinal pathological change which accounts for46.8%(22eyes). The main pathological change is retinal detachment which occurs in37eyes (77.1%). As to teenager patients, proliferative vitreous retinopathy (PVR) before operation has high occurrence with36.1%and posterior detachment of vitreous (PVD) has the relatively low occurrence with14.5%. Retinal detachment has the wide scope.62.1%of the patients have the scope of more than two quadrants and59.4%of patients have the scope of involving macula and67.5%have the scope of bitamporal retina. There are9cases of lentectomy and/or combined with IOL implantation during operation. After statistical analysis it has no influence to prognostic eyesight. Do analysis to retinal detachments scope, its involvement of macula and times of operation and prognostic eyesight. It is shown that it has no remarkable influence to prognostic eyesight.Conclusion:1. The main reason for teenager patients to have pars plana vitrectomy (PPV) is repair kinds of retinal detachment and accompanied PVR with different degrees.2. As to whether teenager’s PPV operation is combined with crystalline lens disposal, it shall be chosen carefully according to the specific situation. If it is expected that many operations will be done to finish eye anatomy and function repair, we shall choose to do the second or third operation as far as possible.3. The main influencing factors of future prognostic eyesight are retinal detachments scope, whether it involves macula and times and frequency of operation.4. In most cases, primary diseases caused by various pathological factors have complex situations. The detachment area is wide, it is very difficult to replace by operation and the result is not so good. Traumatic vitreoretinal diseases operation has the precise effect and the prognostic eyesight is relatively ideal.
