BackgroundWith the development of technology and raise the level of medicaltreatment, total hip arthroplasty common used for various hip diseases, italso led to the cases of revision of total hip arthroplasty increases year byyear. Due to infection, loosing,fractures and so on, leading to the firstTHA failure and patients suffer. And because the reason diversity,anatomical structure change, lead to the revision surgery is difficult,surgery high technical requirements and is orthopaedic surgeons to befaced with difficult problem.ObjectiveTo evaluate the short-term clinical effort of the revision of total hiparthroplasty through the review and analysis of the18cases(20hips)which performed in department of orthopedic of china-japan unionhospital of jilin university from February2008to august2011.Data and methodsGeneral information:from February2008to august2011,18cases(20hips)did the the revision of total hip arthroplasty in department oforthopedic of china-japan union hospital of jilin university.9cases(10hips)were male,9cases(10hips) were female,average age54.5years old(rangefrom28to77years old).10cases are right,6cases are left and2cases arebilateral.The initial reason:14cases(16hips) are osteonecrosis of femoralhead;3cases(3hips) are trauma;one case(1hip) is developmentaldysplasia of hip.The average time of the revision to the initial surgery is 7.43years(from3months to21years).The way of the initial operation:15cases(17hips) are THA;2cases(2hips) are bipolar semiarthroplasy;onecase is surface replacement of femoral head.The prosthesis type of initialoperation:7cases(8hips) are cemented;8cases(9hips) are uncemented;3cases(3hips) are complex.The reason of RTHA:10cases(12hips) areprosthesis loosing;2cases(2hips) are infection;3cases(3hips) areperiprothetic femur fracture;one case is break of prosthesis;one case ispoor function;one case is dislocation.The ways of all revision are THA,10hips are uncemented;4hips are cemented;6hips are complex.Methods:All of the patients using THA for the revision, respectively,in the preoperative and postoperative follow-up of patients use Harrisscore hip functional score, with the help of SPSS, compared thepreoperative of Harris score with postoperative follow-up, evaluation theshort-term curative effect of RTHA.ResultsThe average follow-up time of18cases was14.6months(range from3months to45months).the preoperative average harris score is34.14(range from0to65.75) and the postoperative average harris score is81.07(range from57.15to95.15),all have significantlyimprovement.According to the harris score,among them,4hips areA(100-90),9hips are B(89-80),4hips are C(79-70),3hips are D(<70).ConclusionThe revision of total hip arthroplasty is the most effective treatmentfor the initial total hip arthroplasty failure.Adequate preoperativepreparation,pointed treatment plan,appropriate prosthesis are all veryimportant for the postoperative function and quality of patients’ life.
