BackgroundWith the artificial hip arthroplasty improvements in technology and development of product, the number of Total Hip Arthroplasty underwent by people is gradually increased. Around the world there are nearly one million cases every year, and the working life has been prolonged, even the survival rate of15-20year is still around90%. But due to the increased number of THA, the number of the revision of THA also has increased year by year, there are about10%patients receiving the revision. There are many reasons for revision of Total Hip Arthroplasty, such as infection, periprosthetic fractures, prosthesis fracture and aseptic loosening, dislocation, malposition and other related factors. The revision of Total Hip Arthroplasty not only burdens the patients and their families with physiology, psychology, and economy, but also changes the anatomy levels and disorder the local tissue, which increase the difficulty of revision and surgical technique. That is the major issue all orthopedic surgeons must face to.ObjectiveIn order to evaluate the clinical effort of the revision of Total Hip Arthroplasty, we collect the20cases (22hips) which performed in department of orthopedic of China-Japan Union Hospital of Jilin university from December2008to December2012, and make clinical analysis, and post-operative patients were followed up by using Harris score. At last we briefly discuss the renovation of surgery related to the important and difficult.DataFrom December2008to December2012,20cases (22hips) did the revision of Total Hip Arthroplasty in department of orthopedic of China-Japan Union Hospital of Jilin University. Detailed patient data are shown in Table1. All the patients’average age is53.75years old (range from28to77years old). The average time of the revision to the initial surgery is85.05months (from1month to21years).MethodsAll of the patients under general anesthesia in the original approach and appropriate extension of the approach using THA for the revision, respectively, in the preoperative examination and postoperative follow-up of patients use Harris score hip functional score to evaluate the function recovery of hips, with the help of SPSS, compared the preoperative of Harris score with postoperative follow-up of Harris score, evaluation clinical curative effect of RTHA.ResultsThe average follow-up time of20cases was25.05months (range from2 months to60months). The preoperative average Harris score is33.53(range from0to65.75) and the postoperative average Harris score is80.06(range from57.75to99.25), compared postoperative follow-up of Harris score with the preoperative of Harris score, all patients have significantly improvement. Statistically significant (p<0.05) based on the results of analysis of preoperative and postoperative scores. According to the Harris score, among them,5hips are A,8hips are B,2hips are C,7hips are D.ConclusionThe revision of Total Hip Arthroplasty is the most effective treatment for the initial Total Hip Arthroplasty which failure cause by any reasons. According to perfect preoperative preparation, and physiological and economic condition of patient, we should choose individualized treatment plan, and select the appropriate prosthesis on the basis of the condition getting from the operative, which is a necessary condition for revision surgery success. Post-operative rehabilitation exercise is also another key point to clinical effort and should not be overlooked.
