The water quality of Huai River has been getting worse and worse because of the dramatic population growth, economic development and the rapid acceleration of urbanization. And various types of wastewater from30cities were poured into the river without any sewage disposal, especially the Shaying (SY) River. The water pollutants of SY River have caused great harm to the health of surrounding residents resulting in multiple high-incidence area of cancer around the basin.The incidence of cancer is a muti-stage process. The internal environment had changed absolutely before the occurrence of precancerous lesions, these changes may be the carcinogenic risk factors, which caused non-specific changes of the residents in a short stage,. Their long-term existence in human body has great harm to human health, such as causing the occurrence of cancer. Therefore, the three section levels of residents in cancer high-incidence area along SY River needed to be measured and analyzed to determine whether those could be used as the early detection of cancer targets and then provided scientific basis for later process.Objective:Selecting30-60-year-old residents in less than5km and more than20km away from SY River as research objects in a town, Henan province based on the review of outside environmental testing data and analysis on the Death Cause of inhabitants in Disease Surveillance. To measure and evaluate the HP infection of two districts, the levels of various immune cytokines and apoptotic factors of residents to understand the interaction of these risk factors leading to high incidence of cancer and to explore the impact of SY River pollution on health status of residents.Methods:1.30-60-year-old residents in two countries were chosen as the investigators. According to the strict inclusion and exclusion criteria, a total of786villagers were selected, of which there are312in contaminated areas, while474in control area. Unified trained investigators conducted one-on-one-access questionnaire to all residents and collected their10ml fasting venous. Based on the results of questionnaire, all the subjects were divided into four groups including high risk group in contaminated area, normal population in contaminated area, high risk group in control area and normal population in control area.65residents in each group were randomly selected using random number table.2. To determinate the infection of HP in serum among the residents in these four groups along SY River using immunoblotting, and to measure the type of HP based on the different toxins secreted by cells.3. According to the test results, the levels of cytokines (IL-2, IL-2R, IL-4, INF-y) in serum of residents were measured via enzyme-linked immunosorbent method (Elisa), and comparative analysis were used among these four groups.4. According to the test results, the levels of apoptotic factor (Fas) and apoptosis factor ligand (FasL) in serum of residents were determinate by Elisa to evaluate the apoptosis situation of residents along SY River.5. The statistical analysis. Using statistical software SPSS12.0, the normality test and Levene homogeneity of variance test were used to detect the homogeneity of variance. Statistical analysis referred to Bonfferoni test for multiple comparisons when significant differences were detected by one-way analysis of variance (ANOVA). Besides, SNK test, chi-square test and simple linear regression were also chosen to test the data from experiments and the questionnaire.The difference at P<0.05was considered statistically significance.(Inspection level a=0.05)Results:1The concentration of outside environmental indicators in contaminated area was measured in SY River Basin, which showed that the contents of pollutants had already exceeded Class V in surface water quality standard. The concentration of many heavy metals in contaminated area was significantly higher than those in the control area. The Death Cause of inhabitants in Disease Surveillance showed that cancer incidence was significantly higher in the contaminated area.2. The infection and types of HP in serum were measured among the residents in contaminated high-risk group, contaminated normal group, control high-risk group and control normal group, which were fit to approximate normal distribution. Levene homogeneity of variance test was used to detect the homogeneity of variance. Exact probabilities, R*C and four tables chi-square tests were used to determinate the difference among groups, which provided the basis for the high incidence of gastrointestinal disease.3. The levels of cytokines (IL-2, IL-2R, IL-4, FNF-γ) in serum of residents were measured in contaminated high-risk group, contaminated normal group, control high-risk group and control normal group, which were fit to approximate normal distribution. Levene homogeneity of variance test was used to detect the homogeneity of variance. One-way analysis of variance (ANOVA) and SNK test were used to determinate the difference among groups, which provided the basis for the immune status of residents along SY River.4. The levels of apoptotic factor (Fas) and apoptosis factor ligand(FasL) in serum of residents were determinated in contaminated high-risk group, contaminated normal group, control high-risk group and control normal group, which were fit to approximate normal distribution. Levene homogeneity of variance test was used to detect the homogeneity of variance. One-way analysis of variance (ANOVA) and SNK test were used to determinate the difference among groups, which showed that the apoptotic ability of residents in contaminated area had come down.Conclusions:1. Heavy metals and other pollutants in SY River had polluted the drinking water, diet and vegetables, which could increase the within load. By this way, it can affect the health of residents.2. The infection rate of HP was significantly higher in contaminated area along SY River, especially type I HP, which may be one of the main reasons for the high incidence of gastrointestinal cancer. Some measures should be taken to reduce the infection rate of HP to cut down the incidence of gastrointestinal cancer.3. The immune (the levels of IL-2、IL-2R and INF-γ were significantly higher, but the content of IL-4was lower) and apoptotic(the levels of Fas and FasL were significantly higher) functions of residents in contaminated area were affected by pollutants along SY River. There was a drift of the Thl to Th2cells and the balance disorder between apoptosis and proliferation, by this way, both of them could be regarded as the indicators of early detection of cancer so that some intervention measures should be taken to reduce the risk of tumor development.
