| Object ive:To study the clinical efficacy and safety of Buyanghuanwu decoction in prevention of deep venous thrombosis (DVT) of artificial joint replacement. And to observe the dynamic changes of D-dimer in old patients after hip artificial joint replacement with Buyanghuanwu decoction, thus providing clinical evidence for Buyanghuanwu decoction in prevention of deep vein thrombosis after hip arthroplasty.Methods:60eases qualified in patients all in NO.3orthopedics department of Hospital of Guangzhou TCM from Aug,2011to Apr,2012were divided into TCM group30cases and Western medicine group30cases. Buyanghuanwu decoction and Fraxiparine were applied respectively in two groups after the same basic and mechanical preventive procedure. The results between two groups were analyzed statistically which include incidence rate of DVT. The main observing indexes are Plasma D-dimer levels on Preoperatively day1and Postoperatively days1to days9, the limb swelling ratio on Postoperatively days1,9and bleeding volume during operation and post op-drainage. The results between two groups were analyzed statistically with SPSS13.0.Results:Five cases from60occurred DVT, the rate is8.33%(TCM group:2cases/6.67%;Western medicine group:3cases/10%);There were no statistical significance in DVT incidence rate.D-dimer levels were significantly raised in all patients of the two groups after hip Arthroplasty operation. The elevated levels followed a similar trend to that a peaked distribution with peaks at days1, and then went down at other days. There was no statistical difference between the Preoperative D-dimer levels in our study groups(P>0.05)while obvious difference has been found between postoperative D-dimer levels of the two groups(P<0.05). There was no statistical difference between limb circumference levels on preoperatively day l,and limb swelling Ratio on postoperatively days1,9also has been found no statistical difference of the two groups(P>0.05). There was a significant difference of two cases incision drainage(P<0.05). The TCM group happened one person with bleeding phenomenon, but five persons for the Fraxiparine group, there was no hemorrhage seriously and other side effects in all eases.Cone l us ion:Buyanghuanwu decoction can obviously reduce the Plasma D-dimer levels after hip Arthroplasty in old patients so that it can reduce the incidence of lower limbs deep vein thrombosis. So Buyanghuanwu decoction can significantly reduce the occurrence rate of DVT. And hemorrhage and other side effects was little. It is efficient and safe to use Buyanghuanwu decoction in preventing Deep vein thrombosis(DVT) after hip Arthroplasty. |
