Shoulder-hand syndrome is one of the common complications occurring in patients with hemiplegia following apoplexy. It is typical manifested by pain in the shoulder, edema and pain and ascendant temperature of upper hand, without correct treatment, it will lose the function of the hand which leads to severe disability.There are rarities of treatmeat for the shoulder-hand syndrome. According to the reports of recent years, for the pathogenicmechanisms are not yet clear, the western symptomatic treatment is limited, and its long term therapeutic effect with the effective method of treatment has not yet been proved to be stable, so early rehabilitation therapy is the main treatment method at this moment. Compared with the western medicine, Chinese medicine has great advantage. Contralateral needing is a special traditional acupuncture therapy, based on the function of the channels witch go through both left and right side, by working on the points on contralateral side of the body,it can promote the circulation and bablance of the qi and blood of the both sides. By this way, edema and pain can be eliminated with the methods of contralateral needing and rehabilitation.ObjectiveTo investigate and compare the clinical efficiency of the patient of stroke-induced shoulder-hand syndrome by contralateral needing plus rehabilitation and common acupuncture plus rehabilitation.MethodsSixty patients were randomly divided into two groups, each with thirty cases. Based on the same treatment of rehabilitation, contralateral needing was applied in the treatment group, and common acupuncture was appied in the observation group. The therapeutic effects, pain of the hand, function of upper limb, ability of daily life were objectived after three weeks, and the statistics analysis was did after that.Results1. Therapeutic effects comparion:The numbers of patients with curative, significant effect, effect and no effect were9,13,5and3in treatment group, and the total effect was90.00%;while those of observation group were4,11,7and8, and the total effect was73.33%. Compared the two groups by rank sum test,u=2.1365, p=0.0326, there was statistical significance.2. Observation indices comparion:Pain of the hand:the score of VSA of treatment group and observation group before treat were7.52±1.87and7.34±1.69, while there were2.11±1.23and3.67±1.38after treatment. Function of upper limb:the score of Fugl-Meyer of treatment group and observation group before treat were23.12±5.98and22.56±5.98, while there were48.78±5.34and34.50±7.38after treatment. Ability of daily life:the score of ADL of treatment group and observation group before treat were54.36±13.41and55.12±14.62,73.67±12.90; while there were73.67±12.90and64.34±8.57after treatment.According to the datas, the pain of the hand of the two groups were both reduced, the functions of upper limbs and the ability of daily life of the two groups were both rised. By t test, not only the comparions of each observation indice themselves but the comparions between the two groups before treatment or after treatment were were significant difference. P<0.05, there was statistical significance.It means that both of the two methods were effective treatment methods,but the therapeutic effects of treatment group was much better than the one of the objective group’s.ConclusionCompared with common acupuncture, contralateral needing and rehabilitation had good effects on priod I stroke-induced shoulder-hand syndrome, and there was more apparent improvement in pain, edema, joint movement and the function of the hand.
