Diabetic nephropathy is the commonest and most frequent complication ofdiabetic patients, and a type of the diabetic microangiopathy. Reduced renalfunction induced by this disease, which worsens into renal failure, has becomeone of the causes of the death of diabetic patients, and impacts several systemsof the body. Atherosclerosis is the basis of proteinuria of diabetic patients.Microalbuminuria is active to the abnormal leaky protein from the kidney, anda positive indicator of the early kidney function change. Ankle-brachial index isalso a sound no-invasive treatment of peripheral arteriosclerosis.So it’s a greatsignificance to prove the relationship between Ankle-brachial index andmicroalbuminuria,which plays an important part in medical and social fields.Objective:1.To compare the differences of the normal group and reducedgroup between the patients of gender, age, clinical course, fasting bloodglucose(FBG), triglycerides (TG), total cholesterol (TC), and microalbuminuria(MA).2.To study the correlation with ankle-brachial index and find the way toprevent and early discover diabetic nephropathy, improve the quality ofsurvival patients.3.To provide new ideas and new measures for the diagnosisand treatment of diabetic nephropathy.Material and method:136patients were selected from March2011toNovember2011diagnosed as type2diabetes according to1999WHO diabetesmellitus diagnostic criteria,age:59.4±9.8,clinical course:7.6±9.9.Thepatients were divided into two groups based on the ankle-brachial index (ABI)results,52cases in ABI reduced group(ABI <0.9) and84cases in ABI normalgroup (0.9≤ABI <1.4). Exclusion criteria:(1) type1diabetes patients.(2)with other kidney diseases or metabolic diseases.(3) with many interferentialfactors. Using the United States Summit Doppler Systems company production of ultrasonic blood vessels diagnosis system Vista AVS to measureABI of the patients.To study the differences between the normal group and thereduced.Outcome measures: age, clinical course, fasting blood glucose (FBG),triglycerides (TG), total cholesterol (TC), and microalbuminuria (MA).Analyzing the data between the two groups. The data were expressed as (x±s)and the mean between the two groups were compared using t test. The countingdata between the two groups was compared by x2test. And make out themultiple linear regression analysis.Result:The number of the patients in the reduced group accounts for thepercentage of38. The differences of gender, age, FBG, TC and TG levelbetween the two groups was no statistical significance(P>0.05),while thedifferences of clinical course and MA between the two groups was statisticallysignificant (P <0.05). The clinical course and MA in ABI reduced group weresignificantly higher than that in the normal group,which means the correlationwas remarkable statistically significant. The statistical analysis showed thatclinical course and MA produced a negative influence on ABI, that is, with theextension of duration of diabetes,reducing ABI and with the increase of MA,ABI reduced.Conclusion:The reducing of ABI has a negative correlation of clinicalcourse and MA,and has no no significant correlation with gender, age, FBG,TC and TG level. Early detection of the patients with diabetic nephropathy byABI has a very important significance of the prevention and control of diabeticnephropathy.
