[Abstract] Objective1. To know bacillary dysentery epidemic characteristics, Shigella serotypes and the changes of epidemic strains;2. To understand its resistance for antibiotics and changes of Shigella bacteria.3. To explore the molecular type of Shigella bacteria and find out the optimum type;4. To set up strains database and explore popular strains change situation so as to provide the basis for clinical rational use of antibiotics and the control of it epidemic in Qinghai province.Methods1. To select1city and3rural areas as surveillance sites, information of diarrhea patients about epidemic and laboratory materials were collected. Descriptive epidemiological analysis was conducted.2. Through blood culture, biochemical identification, isolation and identification of the positive strains to explore the serum and strain type according to the requirements for specimens in laboratory testing so as to understand bacillary dysentery of dominant strains within5years of continuous surveillance in Qinghai province;3. The strains isolated from the United States was taken sensitivity test by K-B agar method according to the clinical standards board (NCCLS).Altogether9kinds of antibiotics sensitivity test were done commonly used clinically to understand the drug sensitive degree of bacillary dysentery in Qinghai province for clinically rational application of antibiotics.4. Using pulsed field gel electrophoresis method to on the isolated strains to find out the optimum type and make clustering analysis.Results1. Incidence of bacillary dysentery patients mainly occur in summer and autumn among0-5year group scattered children with more men over women from2005to2009in Qinghai province.2. The main pathogens is group B of Shigella flexneri, which account for78.80%; followed by group D song Neizhi Shigella, which account for21.20%. The group B of Shigella flexneri isoforms in serum were detected into11subtypes, subtypes in Fx variants, which account for62.76%; followed by F4a, which account for14.48%.3. The results of drug sensitivity test:Freund is sensitive to gentamycin, cefotaxime ciprofloxacin is resistant to nalidixic acid, cotrimoxazole and tetracycline; Sonnei is sensitive to ciprofloxacin, cefotaxime, ampicillin and is resistant to amoxicillin, nalidixic acid and cotrimoxazole resistance; wherein, cephalothin is mildly resistant to Fu Salmonella, the resistance rate was2.76%, with significant difference (X2=12.97, P<0.01)to Salmonella; gentamicin is resistant to Freund’s bacteria and the resistance rate was8.97%with significant difference (X2=70.06, P<0.01) while the rest of antibacterial drugs with no significant difference between the two rates of resistance.4. PFGE type were done on111strains,41genotypes were identified including Freund’s fungus33, sonnei bacteria8genotypes, SQHN11.0002including a total of26isolates, which accounts for63.41%of the total belt type and be advantage type in2006and2007in Qinghai Province.Conclusion1. the scattered children and farmers is focus groups for prevention of dysentery in Qinghai Province;2. B group of Shigella flexneri is main epidemic strains of serotype which is mainly based on Fx in Qinghai province;3. Cefotaxime and ciprofloxacinare the optimum drug for treating bacillary dysentery in Qinghai province;4. Different type of PFGE band has little difference, which indicates a less genetic variation in Qinghai province...
