| OBJECTIVE To investigate the distribution and drug sensitivity of pathogens in children with bacterial diarrhea in Kunming,and to understand the pathogen composition and drug resistance.METHODS We collect the clinical data of children with diarrhea and positive stool culture from January 2015 to December 2019,and the results of pathogen and drug sensitivity test were statistically analyzed.RESULTS: 1.Analysis of pathogen distribution in 273 children with bacterial diarrhea:(1)age and gender distribution: there were 273 children with positive fecal culture,including 156 males and 117 females,The ratio of male to female was 1.33:1.The age of onset is mainly 0-3 years old,and 141 of them were under 2 years old,accounting for51.6%;(2)The seasonal distribution of 273 children with bacterial diarrhea from January to December was 7 cases,5 cases,10 cases,24 cases,27 cases,41 cases,37 cases,32 cases,39 cases,23 cases,19 cases,and 9 cases.The incidence occurred throughout the year,and the peak of the incidence was from June to September each year,presenting a unimodal distribution;(3)pathogen distribution among 273 children with positive fecal pathogens,176 cases(64.5%)were infected by Salmonella,including 65 cases(36.9%)of Salmonella typhimurium,35 cases(19.9%)of Salmonella paratyphi B,35 cases(19.9%)of Salmonella unidentified,30 cases(17.0%)of Salmonella typhi,5 cases(2.8%)of Salmonella paratyphi C,2 cases(1.1%)of Salmonella paratyphi A,2 cases(1.1%)of Salmonella cholerae suis,and 2 cases(1.1%)of Salmonella typhi There were 1 case of Salmonella Porter,accounting for 0.5%,1 case of Salmonella enteritidis,accounting for0.5%;89 cases of Shigella infection,accounting for 32.6%,including 50 cases of Shigella sonnei,accounting for 56.2%,36 cases of Shigella flexneri,accounting for40.4%,2 cases of Shigella dysentery,accounting for 2.2%,1 case of Shigella baumannii,accounting for 1.1%;7 cases of Pseudomonas aeruginosa,accounting for 2.6%;1 case of Klebsiella pneumoniae,accounting for 0.4%;2.Drug sensitivity test :The drug resistance rate of Salmonella typhimurium was gentamicin 100.0%(65 / 65),amikacin 98.5%(64 / 65),cefazolin 96.9%(63 / 65),ampicillin 86.2%(56 / 65);The drug resistance rate of Shigella sonnei was cefazolin 100.0%(50 / 50),ceftriaxone98.0%(49 / 50),amikacin 98.0%(49 / 50),gentamicin 96.0%(48 / 50);The drug resistance rate of Shigella flexneri was cefazolin 100.0%(36 / 36),ampicillin100.0%(36/ 36),gentamicin 94.4%(34 / 36),amikacin 91.7%(33 / 36).CONCLUSION:1.The incidence of bacterial diarrhea in children in Kunming was mainly concentrated in infants under 2 years old,while Shigella sonnei infection was mainly distributed in children 3-6 years old;2.The incidence was all year round,and the peak of the incidence was from June to September of each year,showing unimodal distribution;3.Salmonella and Shigella were the main pathogenic agents,Salmonella typhimurium and Shigella sonnei were the main pathogenic of bacterial diarrhea;4.Compared with the pathogens of bacterial diarrhea in Kunming 10 years ago,the resistance rates of Salmonella typhimurium,Shigella sonnei and Shigella flexneri to many antibiotics were different,the main pathogens and antibiotic resistance had changed. |
