ObjectiveTo discuss the significance of combined use of paternally imprinted gene product p57Kip2immunohistochemistry and DNA ploidy analysis in the differential diagnosis of placental hydropic diseases.MethodsA total of35cases of hydropic placenta were collected, from the Department of Pathology, The First Hospital, Shanxi Medical University, during2007-2009. Immunohistochemical staining (Envision) was used to detect the expression of p57Kip2in22CHM,7PHM and6HA. All cases were examined by DNA ploidy analysis. Through the comparison of the three methods (HE methods, immunohistochemical staining and DNA ploidy analysis), to investigate assistant diagnostic value and significance of the hydatidiform mole.Results1. p57Kip2was detected in the nucleus of langhans cell and bloom stromal cells by Immunohistochemistry. Expression of p57Kip2in HA, PHM and CHM was respective-ly for100%,85.7%,18.2%. Expression of p57Kip2between PHM and CHM, CHM and HA exist significant differences. There was no statistic significance between PHM and HA(P>0.05).2. DNA ploidy analysis showed that all of7PHM were triploidy,20of22CHM were diploidy or pseudodiploid(90.9%),4of6HA were diploidy(66.7%).DNA ploidy analysis between CHM and PHM, PHM and HA exists significant differences. There was no statistic significance between CHM and HA (P>0.05).3. Results of p57Kip2immunohistochemistry and flow cytometry:p57Kip2was negati-ve and DNA was diploid for18CHM. p57Kip2was positive and DNA was diploid for6HA, p57Kip2was positive and DNA was triploid for10PHM. Compared with the result of immunohistochemistry,1case showed p57Kip2negative, but DNA ploidy analysis was triploid, so the diagnosis was corrected to the PHM. The difference between each group was no statistic significance(P>0.05). Conclusions1. p57Kip2negative is the reliability makers of the CHM. It can be used as the auxili-ary diagnosis method to identification CHM and non-CHM. For PHM and HA identif-ied sometimes turn to other methods.2. Flow cytometry DNA ploidy analysis helps to identifying the triploid PHM and the diploidy CHM, HA. Both CHM and HA showed diploidy, so the DNA ploidy ana-lysis is not helpful for differential diagnosis between them.3. Diagnosis by combined use of p57Kip2immunohistochemistry and flow cytom-etry helps to make a clear distinction between three kinds of placental villus edema lesions.
