Objective: Hydatidiform Mole is one of the most common diseases ingestational trophoblastic disease, divided into complete hydatidiform mole andpartial hydatidiform mole. Hydatidiform mole is a benign disease, but withsome malignant tendency, complete hydatidiform mole is about6%-32%(CHM) and partial hydatidiform mole is about5%(PHM) will developgestational trophoblastic tumor[1].The majority of patients with hydatidiformmole after curettage can be cured, but the mole with active growth andpotentially malignant characteristics, complete hydatidiform mole cancerationrate is about20%, Currently determining the prognosis of patients withhydatidiform mole by the follow-up after curettage, observed changes andclinical characteristics of patients with serum β-HCG, need at least eightweeks to find malignant, But some patients because of lack of comprehensiveunderstanding of hydatidiform mole or because of medical conditions of eachregion is not perfect not regular follow-up, these patients often appear inclinical symptoms before treatment, so that the delay treatment, to developadvanced malignant and life-threatening. Because there is no reliable predictorof hydatidiform mole and the malignant transformation, so looking forrelevant predictor of hydatidiform mole malignant change has importantclinical significance.In this study, through the expression of determination of CD147andMMP-2in hydatidiform mole tissues, to explore the two development andmetastasis in hydatidiform mole, and relationship between the two, todetermine whether it can become an important index to predict the malignantchange of hydatidiform mole.Methods:Specimen collection and processing:30cases were collected from patients with hydatidiform mole who received initial treatment in the ThirdHospital of Hebei Medical University between November2005and July2012.30cases were pathologically confirmed as hydatidiform mole. And thecollection of normal pregnancy (6-8weeks) without other diseases, toterminate the pregnancy villus tissues of20cases.take the first evaluationorganization, with4%of the neutral formaldehyde soak, rinse with water,100%,90%,75%of the gradient alcohol dehydration, xylene transparent,paraffin embedding. Continuous paraffin section, thickness of0.5um.Grouping:The organization is divided into normal villi group (20cases),hydatidiform mole group (24cases), invasive hydatidiform mole (3cases)to study the expression and significance of MMP-2,CD147in each group.Then the mole in the high risk group (23cases) were divided into malignantgroup (3cases), not the malignant group (20cases) further research onMMP-2, CD147expression in hydatidiform mole patients with high riskfactors and with carcinomatous change of hydatidiform mole tissue.Test method: Paraffin sections after conventional dewaxing, rinse3times,repair liquid in95℃hot water bath for10minutes in repair. Place the roomtemperature to repair liquid temperature dropped to37℃below, rinse3times,3%H2O2solution at37℃deposited fertile. Rinsing with distilled water, PBSfor5minutes. Added to rat anti-human MMP-2, CD147monoclonalantibody to an anti-MMP-2concentration of1:200and CD1471:200,4℃overnight. Clean water three times, added to rat anti-human secondaryantibody was incubated at37℃40min; rinse with water three times. Addfresh configuration of DAB reagent, observed under the optical microscope,dyeing, timely termination of dyeing. Hematoxylin counterstain;differentiation of blue, dehydrated, transparent.Neutral resin sheet, in under alight microscope results judgment.Detection method: Each slice randomly selected five horizons,the imageprocessing system with HMIAS-2000statistical gray scale value, the averagegray value as biopsy results, gray value lower protein more.Statistical analysis: Data were analyzed by SPSS13.0software, compared with the expressionintensity of CD147, MMP-2in invasive mole, mole and normal villi of twoindependent samples t test, P<0.05indicates that there are statisticallysignificant differences.The correlation between MMP-2, CD147using Pearsonproduct-moment correlation, P<0.05shows significant differences.Results:1Normal early pregnant chorionic villi morphology under microscope:syncytiotrophoblast and cytotrophoblast cells arranged in neat villi, atypicalcells less.2Hydatidiform mole morphology under microscope:(1)Villous stromaedema formation of the large central pool.(2)Trophoblastic hyperplasia:Syncytiotrophoblast hyperplasia with abnormity of certain,cytotrophoblastcells arranged more orderly and two kinds of cells are often mixed together.3Expression and localization of MMP-2: MMP-2expression insyncytiotrophoblast cells,and cytotrophoblast cells,cytoplasm, a small amountof expression in syncytiotrophoblast cells,and cytotrophoblast cells,membrane.4Expression and localization of CD147: CD147expression insyncytiotrophoblast cells, and cytotrophoblast cells, membrane, a smallamount of expression in syncytiotrophoblast cells, and cytotrophoblast cells,cytoplasm.5The results of MMP-2determination: MMP-2showed a decreasingtrend in the expression of invasive hydatidiform mole group, hydatidiformmole group, normal villi group, and the difference between the two groupswas statistically significant.6The results of CD147determination: CD147showed a decreasing trendin the expression of invasive hydatidiform mole group, hydatidiform molegroup, normal villi group, and the difference between the two groups wasstatistically significant.7MMP-2and CD147relationship: aggressive hydatidiform mole group,hydatidiform mole group and normal villi of MMP-2expression and the expression of CD147has certain correlation, were positively correlated.8The average gray value of MMP-2in the malignant group with riskfactors than not the the malignant group ’s average gray value, and thedifference was statistically significant.(P=0.008P<0.05)9The average gray value of CD147in the malignant group with riskfactors than not the the malignant group ’s average gray value, and thedifference was statistically significant.(P=0.010P<0.05)Conclusions:1MMP-2, CD147in normal villi tissue, hydatidiform mole, Aggressivehydatidiform mole expression in hydatidiform mole, and expression intensityshowed a trend of gradual lifting and MMP-2rises as the CD147expressionwere positively correlated, Speculated that high expression of MMP-2, CD147may occur in hydatidiform mole development process, and may promote themalignant transformation of hydatidiform mole.2In hydatidiform mole containing risk factors in patients with malignantchange the expression of MMP-2and CD147obviously higher inhydatidiform mole group, prompt combined detection of MMP-2and CD147in predicting malignant transformation in hydatidiform mole is of clinicalsignificance, has the potential to be clinically important predictor ofhydatidiform mole malignant change.
