Purpose:We retrospectively reviewed cases to determine the impact of age and gender on the clinicopathological characteristics of urothelial carcinoma, which contribute to selecting the appropriate treatment protocols. And we further investigated the relevant factors affecting the treatment of disease.Methods:From July2000to August2011, the clinicopathological records of913patients with urothelial carcinoma were retrospectively reviewed. The age was analyzed both as a numerical and categorical variable. According to their age, the patients were classified into four groups:<40years group,40-59years group,60-79years group,=80years group; according to their gender, the patients were classified into two groups:male group and female group. The groups were evaluated for pathological stages, pathological grades and treatment.Results:In the913patients,82.7%of the patients presented with painless hematuria. The mean age was63.5±12.6years old (range:20-97years old) and the male to female ratio was4.6:1;26.3%of the patients was muscle invasive urothelial carcinoma at the time of presentation. In patients with nonmuscle-invasive urothelial carcinoma,63.4%of the patients was Ta disease while10.3%of the patients was T1disease;64.8%of the patients was low-grade urothelial carcinoma,35.2%of the patients was high-grade urothelial carcinoma. Among the age groups, the male/female ratio in the four age group was as follows:3.0:1.0in<40years group,5.8:1.0in40-59years group,4.5:1.0in60-79years group,3.3:1.0in=80years group. There was no significant difference in the male/female ratio among the four groups. There was a significant association between age (both as a numerical and a categorical variable) and the clinicopathological characteristics of urothelial carcinoma. The percentage of muscle invasive urothelial carcinoma in four groups was:4.5%,18.1%,30.6%,37.7%, respectively, there was significant association among the four groups (P<0.01). The percentage of high pathological grades in the four groups was:6.8%>24.8%n39.7%、55.8%, respectively, there was significant difference among the four groups (P<0.01). The percentage of the treatment of radical cystectomy in the four groups was:9.1%、12.4%、18.6%、13.3%, respectively, there was significant association in the four groups (P<0.01).Overall, the percentage of muscle invasive urothelial carcinoma and the degree of malignancy increased with aging. The incidence of various urothelial carcinoma stages, grades and treatment was no significant difference between male group and female group.Conclusion:There is significant relationship between age and clinicopathological characteristics of urothelial carcinoma. The clinicopathological characteristics of urothelial carcinoma are disparate in the age groups. These features have instructively suggestion on the treatment of urothelial carcinoma. Impact of gender on the clinicopathological characteristics of urothelial carcinoma need to be further researched.
