ObjectiveTo observe the clinical effects and safety of DanHongHuanWu Decoction in thetreatment of cerebral infarction of middle-aged and young patients with highhomocysteine(Hcy), meanwhile to evaluate its interference effect on Hcy.The curative effectof DanHongHuanWu Decoction is evaluated by Clinical symptoms and biochemical indicator,further investigates its mechanism of action and the relationship between acute cerebralinfarction and serum Hcy, Aim to find effective curative medicine for cerebral infarction,so asto extend its clinical application.MethodsThe cerebral infarction of middle-aged and young patients with high Hcy all selecedfrom Hospitalized patients of Affiliated Hospital of Hebei University Emergency Departmentfrom2010October to2011May. For the patients meeting the diagnostic and the inclusioncriteria,64cases were randomly divided into two groups, control group and therapy group,every group consist of34cases. There is no difference in sex、age、pathogenesis、Severity ofillness、concomitant disease and history of past illness between two groups(P>0.05), the twogroups were comparable. The cases of control group were treated with conventional therapyaccording to the patient’s condition, including Oxygen inhalation、reducing intracranialpressure、controling blood pressure、lowering blood lipid and blood glucose、Maintenanceof water and electrolyte balance, antiplatelet therpy(Sodium Ozagrel and Sodium Chloride80mg, two times a day for intravenous infusion, enteric coated aspirin100mg was takenorally after14days, one time a day,continued14days), promoting blood circulation andremoving blood stasis(Ginkgo leaf injection20ml was dissolved into Sodium ChlorideInjection200ml, a time a day for intravenous infusion, after14days, Ginkgo leaf tablet wastaken orally,2tablets every times,3times every day, continued14days),28days is a courseof treatment. Therapy group were treated with conventional therapy and DanHongHuanWuDecoction (Dan shen20g、Honghua15g、Shenghuangqi30g、Chuanxiong15g、Taoren10g、Chishao10g、Dangguiwei10g、Dilong10g、Sangzhi30g、Guizhi15g、Shengheshouwu20g、Shanzha30g、Jixueteng30g,1Ji a day, using water stewed medicines2times,200ml tisane was taken orally at morning and night on an empty stomach,2times every day,).28days isa course of treatment. The treatment effect was evaluated after a therapeutic cycle. Observingneurological functional deficit scale (NFDS)of two groups before and after treatment、 theliving self-care ability and blood plasma Hcy of two groups before and aftertreatment,observing adverse reaction of two groups during medication, the change of bloodcells analysis、Liver and kidney function before and after treatment.Statistical method using SPSS13.0to analyze, measurement data expressed usingmean±standard deviation, two samples were compared with t test. Two groups of count datawere compared with χ2test,ranked data were compared with Ridit analysis test. P<0.05has asignificance difference.Results1. The comparison of two groups in neurological functional deficit scale befoeand after treatment28days.After treatment,The NFDS of control group and therapy group both descended, therapygroup descended more obviously, there was a significant difference compared with twogroups (P<0.01).the results indicate that DanHongHuanWu Decoction could improve nerveinjury recovery for acute cerebral infarction of middle-aged and young patients with highHcy.2.The Comparison of two groups in clinical efficacy after28days treatmentIn the therapy group, the basic recovery patient were eleven cases, significantadvancement patient were sixteen cases, advancement patient were five cases, effective ratewas94.11%. The control group after treatment, the basic recovery patient were eight cases,significant advancement patient were thirteen cases, advancement patient were five cases,effective rate was76.47%.By statistical analysis, there was a significant difference in effectiverate between two groups(P<0.05). the results indicate that DanHongHuanWu Decoction couldimprove symptoms recovery for acute cerebral infarction of middle-aged and young patients withhigh Hcy.3. The comparison of two groups in Hcy content befoe and after treatment28days.The therapy group before treatment, the Hcy content was23.41±4.58mg/L, aftertreatment, the Hcy content was12.93±2.38mg/L;The control group before treatment,the Hcy content was22.94±4.33mg/L, after treatment, the Hcy content was18.26±2.46mg/L.aftertreatment, both group had a decline in Hcy content, therapy group declined more obviously,there was a significant difference compared with two groups (P<0.01). the results indicate thatDanHongHuanWu Decoction could obviously descent blood plasma Hcy for acute cerebral infarctionof middle-aged and young patients with high Hcy.4. adverse reaction.There was no obvious adverse reaction occurrence of two groups during Clinical Trials,further examination blood cells analysis、Liver and kidney function after treatment, there wasno abnormal change related treatment be found, Indicating DanHong-HuanWu Decoction wasinnocuity and no adverse effect.ConclusionThe results of the study indicate that: DanHongHuanWu Decoction could improvesymptoms of acute cerebral infarction of middle-aged and young patients with high Hcy,improving nerve injury recovery, reducing neurological functional deficit scale, decreasingblood plasma Hcy content, By statistical analysis, its curative effect was Significantly betterthan the control group, having a significant difference. At the same. there was no obviousadverse reaction of two groups during Clinical Trials, drug related abnormalities was notfound through observing safety analysis index. DanHongHuanWu Decoction has a bettercurative effect for treating cerebral infarction of middle-aged and young patients with highHcy, our experiment Perfects the mechanism of DanHongHuanWu Decoction, But still needsfurther expansion in the clinical and basic research.
