Objective:Combined with the technologies, such as contemporary image, image processing,reverse engineering, computer aided design and manufacturing technology, the studyhave researched the fabrication, accuracy and clinical application of the digitalimplant surgical templates.Methods:1.The fabrication and accuracy of teeth supported surgical templates: eight pigjaws were CT scanned, the date were imported by Mimics software to reconstruct,then simulating implant by Simplant sofeware. According to the planed implantsposition, we designed the template with3-matic sofeware. Finally the surgicaltemplate were fabricated by RP machine. Under the guidance of the template inedentulate area of pig jaws,8holes were prepared. The jaws were scanned once again,compared the planed position with actual position of the holes in three-dimensionaldirection to evaluate the accuracy of the template.2.Fabrication and accuracy of the mucosa supported surgical templates: accordingto the aforementioned method, four edentulous patients’ CT dates were chosen, thedates were imported in Mimics software to reconstruct. Then the jaw models werefabricated by the rapid prototyping with2mm thick silica gums. Simulating implantsby Simplant sofeware. The guiding template were designed by3-matic sofeware.Finally the surgical template were fabricated by RP machine. Under the guidance of the template in the resin models,30holes were drilled. Then the models were CTscanned once again, compared the planed position with actual position of the holes inthree-dimensional direction to evaluate the accuracy of the template.3.16cases were randomly chosen in fujian medical university hospital affiliatedhospital from January2010to February2012, with oral teeth defect or edentulouspatients. Under the guidance of the template, we placed58implants and evaluated theclinical effect.Results1.Mean deviations of the8holes in the pig jaws were measured, the averagedistance deviations at the entry and apical point of the implant were (0.62±0.18)mmand (0.75±0.19)mm, and the average axis deviation was (1.45±0.44)°.2.Mean deviations of the30holes in the models were measured, the averagedistance deviations at the entry and apical point of the implant were (0.83±0.34)mmand (1.08±0.45)mm, and the average axis deviation was (4.44±1.91)°.3.58implants were placed in16cases undered guidance of the surgicaltemplates, the templates could guide the direction of the implants, the final restorationeffects were satisfactory. Parts of the immediate loading cases, the prosthesiesfabricated in advance were in place successfully by minor adjustments, with goodocclusion.3-12month later, there was not a failure implant, the effects of the implantrestoration were stability.Conclusion:Vitro experiments and clinical cases showed that CAD/CAM template can controlthe position of implants position precisely, ensure accuracy of the implant surgery.
