Purpose. The purpose of this study was to compare the maximal angular deviation between the ideal implant position and the extreme deviated implant position with a Simplant/Surgiguides in a mandible cast model, which can make clinicians aware of the accumulated accuracy of the stepwise drilling due to the drill-sleeve gap.Material and methods. Two methods (stepwise and separately drilling) were performed the implant placement of drills (02.2mm,02.8mm,03.5mm) in partial edentulous plexi-glass models with the drills in a maximal inclination within the sleeves. The angular deviations between the ideal implant position and the actual implant position were measured to evaluate the accuracy of templates. Data was analyzed with a t test (α=.05).Results. Each template's maximal angular deviation demonstrated a statistically significant difference, angular deviations of template A (5.15°±1.27°; 4.32°±1.23°) were higher than template B (1.42°±0.78°; 1.08°±0.33°) and C (3.00°±0.64°; 2.35°±0.81°), which P value< 0.05. And the deviation of template C was higher than B (P (separately drilling)=0.01988<0.05; P (stepwise drilling)=0.02667< 0.05).There was no significant difference between the two drilling methods in maximal angular deviation. The maximal angular deviations of template A and C were significantly higher than theoretical maximal inclination angles (1.798°,1.581°).Conclusions. The accuracy of pilot template which guided pilot drill to the planned place is the key in implant placement of Computer-based guided surgery. The surgery guides firmly secured on the implant bed during the drilling procedure are important for the accuracy translation of surgery guides. In future, the optimum drill-sleeve gap and relation between the implant position deviation with stepwise and separately drilling methods are depended on the further researchs.
