Left-displaced abomasum (LDA) in cows is one of the most common diseases in theintensive dairy farms, and common therapies for them are conservation and laparotomy in ourcountry. Conservation therapy includes rolling and drug, and these two methods are with lowrecovery rate but high relapse rate. There are several kinds of laparotomy, with a highrecovery rate, severe hurt to the cow body and expensive cost, and they may be chosenaccording to the body condition of sickly cows and the preference of veterinarians. It isneeded in veterinary clinical practice that a simple and easily performed therapy for treatmentof LDA is with low cost and slight hurt. In this study, based on the rolling of cow body (Roll),the specific method of operation of toggle pin suture technology was searched and thetechnology was evaluated, which laid groundwork to generalize the use Roll and toggle pinsuture (TPS) technique (Roll-TPS) in veterinary clinical practice.1. LDA cases whose abomasum returned to the normal position after Roll treatment werepunctured with a trocar, then two toggle pins were put into their abomasum through the trocarrespectively, and then a knot was tied with threads of the two toggle pins in vitro to fix theabomasum. The therapy was evaluated by observing recovery of the cases and counting thecure rate after the treatment. The results showed that abomasums in9cases returned to theirphysiological position after Roll treatment for the22LDA cases, that means the success rateof rolling is40.9%, Then the9cases were further treated using TPS immediately, and theywere all cured at last. Thereamong, the success rate after the first toggle pin suture treatmentis88.9%(8/9), and then the success rete is100%after the second toggle pin suturetreatment(9/9). Except the time needed for cure, the performance of the9cases are similar,such as the appetite, rumen motility sounds and milk yield recovery.15days after thetreatment of the rolling and toggle pin suture, the average milk yield of the cases is25.2kgand it is faster than the milk yield recovery of the cases after the laparotomy. This study showthat the toggle pin fixation can strengthen the therapeutic efficacy of Roll treatment forleft-displaced abomasum in cow, it has a little effect on the milk yield and needs low cost, andit is an practical treatment technology in clinic. 2. Seriously analyzing and deeply thinking the above results, we found some remainedquestions, such as how about the harm of toggle pins to the abomasum, whether toggle pinswere in the abomasum after the fixation threads of toggle pins were removed, how about theadhesion of the abomasum and abdominal wall and so on. To make the remained questionsclear, we designed a simulation experiment in goats. Two toggle pins were visually put in theabomasum of a goat through a trocar under open abdominal cavity to fix the abomasum invitro. X-ray test was done at eighth day after abomasum fixation, and the shadows of the twotoggle pins could be found in each abomasum of the three goats. The fixation thread of thetoggle pins was removed at ninth day after the abomasum fixation, the second X-ray test wasdone at thirteenth day after the abomasum fixation, and the shadow of the toggle pin could notbe found in each abomasum of the three goats. When they were necropsied, local adhesionwas found between the abomasum and abdominal wall in each of the three goats, the togglepins were not sought out from each abomasum of the three goats, there were two perforatedtraces in the abomasum mucosa of each goat, and there was slight swell and prominence onthe abomasum mucosa around each perforated trace. The results of this study show that togglepins slightly stimulate abomasum mucosa during Roll-TPS treatment but the toggle pins canbe excreted with the gastrointestinal tract content when the fixation thread of the toggle pinswas removed, and the adhesion formed between the abomasums and abdominal wall canprevent the recurrence of displaced abomasums.In conclusion, the rolling and toggle pin suture technology which is good therapeuticefficacy, low cost, slight damage to the body, small effect of the milk yield, can be used as anoptimization method to treat the left-displaced abomasum in cow in clinical veterinary. But itis suitable for the simple left-displaced abomasum cases in cow without any complication.
