Objectives:Study of fluconazole on the feasibility of prophylactic antifungal therapy inpatients with the sever COPD.Methods:We took a randomized controlled clinical study. Selected100cases of severeCOPD patients were randomly assigned to fluconazole treatment group and controlgroup of50cases. Treatment group, four days after admission was given fluconazolein sodium chloride injection0.2g ivgtt Bid, The control group did not give antifungaltherapy, The two groups at the same time the regular individual treatment of COPD.And two groups of patients on a regular basis sputum fungal culture and did chest CTexamination. Than, we observed the fungal infection conditions and drug-relatedadverse reactions and efficacy.Results:1. Fungal infection of the treatment group was10.20%and27.08%in thecontrol group fungal infection, fungal infection compared with the treatment groupand control group, P<0.05, there is significant statistical difference. The samemeaning, fungle infection can be reduced significantly by prophylactic antifungaltreatment during the COPD treatment process.2. Older than60years of age with fungal infection in the treatment group is13.33%,35.71%in the control group, the two groups compared, P<0.05, there issignificant statistical difference; Younger than the age of60patients, the fungalinfection rate in the treatment group is5.26%,15.00%in the control group, the twogroups compared, P>0.05, no significant statistical difference. The same meaning,fungle infection can be reduced significantly by prophylactic antifungal treatmentduring the COPD treatment process in the older than60patients, but it's not sure inthe younger than60patients.3. During the treatment time, there isn't adverse reactions of the liver and kidneydysfunction, skin rash and gastrointestinal reactions. Conlusions:Fluconazole prophylactic anti-fungal treatment of severe COPD patients iseffective and safe. Especially in elderly patients who benefit significantly.
