Study On Sustainable Development Countermeasures In Construction Of Electronic Health Records Based On Stakeholder Theory

Objective:Define,classify and Sort stakeholders in EHR construction by influence, so as to confirme the core stakeholders;Understand interests pursuit, realize degree, the problem faced, attitude and intention of all stakeholders;Search countermeasures to solve the interests related problem and coordinate the interests relationship, expecting to benefit EHR construction.Methods:Acquaint with related theory and knowledge through literature review method;Determine the type of stakeholders and influence size with Stakeholder Impact Index and Mitchell Evaluation after experts consultation;Investigate interests pursuit, realize degree, the problem faced, attitude and intention through observation of core stakeholders, focus interview and questionnaire survey.Results:(1)EHR construction involved five categories sixteen kinds of stakeholders which consist Health administrative agencies,medical and health service providers,technology service providers,the third party and ultimate beneficiary population.(2)The attribute weight coefficients for stakeholders in EHR construction are:influence value0.435, legitimacy value0.284, urgent value0.281.(3)Stakeholders with top five stakeholder impact index are:health administrative department (Sii value0.825), primary medical and health institutions (Sii value0.588),public health professional organizations (Sii value0.415), software vendors (Sii value0.409) and medical personnel (Sii value0.322),Sii of residents is0.168.Conclusions:(1)Health administrative departments, public health professional organizations, comprehensive hospitals, primary medical and health institutions and medical personnel are deterministic stakeholders;software vendors lack of legitimacy, residents lack of influence, they are expected stakeholders.(2)Health administrative departments, primary medical and health institutions, public health professional organizations, software vendors, medical personnel and residents are core stakeholders in EHR construction.(3)Interests pursuit, realize degree, the problem faced, attitude and intention of stakeholders are not identical.Inconsistencies of Stakeholder consensus and imperfect compensation measures are two core issues in EHR construction.(4)To promote the sustainable development of EHR construction, we should do the following well:①Unite cognition,prompt the parties to reach interest consensus.②Strengthen supporting, increase investment and improve the interest compensating measures.③Coordinate linkage,integrate resources, and make all stakeholders work fully.④Strengthen the supervision of interests to ensure the sustainable development of EHR construction.
