Pituitary adenoma is a common type of brain tumor, accounting for15%of intracranial tumor, only lower than the Glioma and Meningioma in theincidence, and the popμlation's incidence is approximately1/1000. Recentlysome report displayed the incidence of pituitary adenomas is3-5times thanbefore. Invasive pituitary adenoma is often benign in nature, but showmalignant in biological behavior, being invasive of near organization,oppression and wrapping around nerves and blood vessels. Medical treatmenthas poor effect. And surgery treatment is difficμlt for totally excision. To one'sdisappointment, the postoperative disability and mortality rate are extremelyhigh, and easily recurrence. So invasive pituitary adenomas is thought as ahard problem in clinical neurosurgery all the time. Galanin, p21and CD44may play an important role in the invasive growth of pituitary adenomas.Objective: Through examination of galanin, p21and CD44expression atthe RNA level in pituitary adenoma, the thesis seeks to analysis differentialexpression of the three between the invasive group and non-invasive group,and to explore whether there exist any relationship between the three andinvasiveness of pituitary adenomas. This study aims to find new molecμlartreatment rake point for invasive pituitary adenomas.Method: We have collected134cases pituitary adenomas specimen,including male62cases, women72cases, average age (44.9±12.6) age, whichget from Tiantan hospital affiliated to Capital Medical University during2009.4-2011.9.All the above specimens were obtained from patients' freshtumor tissue resected by transphenoid or transcranial microsurgeryrespectively, which quickly be stored in liquid nitrogen after resection.Thenspecimens are divided into5levels according to Knosp grading standards, andgrade0-2into non-invasive group, grade3-4into invasive group(the specimen invasive behavior diagnosis by intraoperation is also included in the invasivegroup).The experiment followed by the line: Trizol one step isolation of totalRNA in all specimens; determination of RNA purity by ΜltravioletSpectrophotometric; examination of RNA integrity by1%agarose gelelectrophoresis; reverses transcription to cDNA by the RT-PCR method;testing specimen galanin, P21, CD44expression at the RNA level by Q-PCRmethod. At last, experimental data is analyzed by SPSS16.0software forstatistical and all data is non-normal distribution. So comparison of thedifference is analyzed by non-parametric tests. It is taken as the significantdifference when p<0.05.Correlation is analyzed by Spearman tests.Resμlt: Real-time fluorescence quantitative PCR resμlt suggest: the ratiobetween galanin,p21,CD44RNA expression and GAPDH is respectively(0.283±0.840,19.647±17.735,7.889±8.378)in invasive group, and that innon-invasive group respectively (0.960±2.694,16.414±13.977,14.956±17.100). Galanin RNA expression in invasive group is clearly lowerthan non-invasive group, and their differences has statisticallysignificant(P=0.037<0.05) by non-parameter test; there is no obviouslydifference between invasive group and non-invasive group of p21and CD44RNA expression, and the value of P is respectively0.511,0.086,P>0.05.Galanin and CD44expression at the RNA level shows highlycorrelated(R=0.370P<0.01) by Spearman correlation analysis.One-wayANOVE indicates that galanin,P21,CD44RNA expression has no apparentlydifference between every levels of pituitary adenomas. Galanin expression isalso no difference in PRL, GH, FSH between the two groups, but obviousdifference in non-function adenoma.Conclusion:1Galanin RNA expression in invasive group is significantly lower than non-invasive groups, which indicate that low expression of galanin may cause theinvasiveness of pituitary adenoma. Then we can specμlate galanin's possibleinhibition of invasiveness in pituitary adenoma, which provide a new directionfor the treatment of invasive pituitary adenoma. 2The expression of P21and CD44shows no difference between invasivegroup and non-invasive group. There may be two reasons:①Expression of thetwo have no correlation with invasiveness of pituitary adenoma.②Samplecannot better reflect the overall situation because the number of specimenfailed to meet the statistical requirements.3The expression of CD44and galanin RNA appears obviously correlation,which displays that possibly CD44is involved in the course of galanin'ssuppression for invasive pituitary adenoma. Galanin, P21and CD44haveexpressed in all types of pituitary adenomas, and its expression has nosignificant difference between at all levels of pituitary adenomas. The threeshow no apparently change with the increase in aggressive tendency ofpituitary adenomas.
