Objective Determine the fluoride in drinking water, the villager's urine, the surrounding environment and compare villager's urine level before and after drinking water improvement project in Hexi, Palang villages, Yuanmo, Yunnan. Monitor the tendency of the fluoride level in urine and drinking water after water improvement project to realize the present issue and then provide science basis and suitable suggestion for the next step.Method Selected Hexi and Palang to be the target site by typical case sampling. Investigate315villagers through group interview. According to national standard, determine the fluoride of villagers'urine, the new drinking water with ion-selective electrode. Analyze the determine data combined with other factors from basic information, irrigation water, crops and related soil by nonparametric statistics, X2, Logistic etc. of SPSS17.0software.Result (1) Among315villagers, the average age is37.0±28.0(M±Q). The main villagers are youth; the proportion of gender in male and female is similar, which is50.5%and49.5%separately. The marriage proportion is71.1%and the largest minority is Han (89.2%). Most of the villagers are elementary and middle school education level.(2)Fist, separating315villagers into two groups, one is children group (age:8-12), the other is adults group (age:over18), after the analyze, neither of them the G(geometric mean, G) of urinary fluoride level reached to national standard(children≤1.4mg/L, adults≤1.6mg/L) before and after drinking water improvement project. After water improvement project in Hexi, the G urinary fluoride level of children group was decreased from2.69mg/L to2.60mg/L and adults group was increased from3.29mg/L to3.57mg/L. In Palang, the G of urinary fluoride level of children group was decreased from2.94mg/L to2.06mg/L and adults group was increased from4.04mg/L to4.81mg/L. Second, the villagers in Hexi and Palang are separated into5groups by different ages. After the determined, in Hexi, all the groups'urinary fluoride have decreased except the oldest group (age:67-83). In Palang, only in7~21group and37~51group the urinary fluoride have decreased.(3)After the Binary Logistic analyze, the old urinary level influence to new urinary fluoride level, which means the higher old level in the past, the higher level now. Children group (OR=12.056,P<0.05),adults group (OR=7.609, P<0.05).(4)After the drinking water improvement project, Hexi's drinking water fluoride level reached the national standard(≤1.2mg/L),it's average of water fluoride level is0.96mg/L and Palang's average of drining water fluoride level is1.29mg/L, which still over the national standard. Both Hexi and Palang, the results have obvious difference after drinking water improvement project P<0.05).(5) The fluoride level of irrigation water in Hexi reached the national standard (≤3.0mg/L) and there were two irrigation water sample reached the national standard in Palang.(6)Among the crops, only a kind of plant reached the national standard (1.Omg/kg).(7)According to national standard(G:0.15mg/kg).None of the soil fluoride level reached the national standard both in Hexi and Palang.Conclusions The time of drinking water in Hexi and Palang is too short and the average of drinking water fluoride level, in Palang, still over the national standard. Part of the crops, irrigation water and the soil were over the national standard both in Hexi and Palang. Some villagers are lack of knowledge to prevent and treat fluoride poisoning, meanwhile, the attitude of villagers to join in the prevent fluoride training was negative.
