Objective:Through observation of relationship between NT-proBNP level and diastolic heart function in patients with atrial fibrillation and its clinical significance, it was hopeful to find out whether insufficiency of diastolic heart function exists in the patients with atiral fibrillation, and discuss the evaluation and application value of the level, and hence an appropriate treatment to the patients.Methods:1. Altogether 80 patients (49 male.31 female) with atrial fibrillation and normal systolic heart function (LVEF>50%) received electrical conversion, and their serum level of NT-proBNP was measured before another electrical conversion. Meanwhile, the serum level of NT-proBNP in 40 healthy people without atrial fibrillation was also measured for comparison.2. One month later, the serum level of NT-proBNP of the patient with twice electrical conversion was measured again, and the variation of NT-proBNP before and after it could be obtained.3. Meanwhile, their diastolic heart function was determined with ultrasonograph, and the relationship between the serum level of NT-proBNP and diastolic heart function could be deduced.4. The variation in level of NT-proBNP and its relationship with patients' diastolic heart function were carfully studied to reach a safe and reasonable conclusion.Results:1. The serum level of NT-proBNP in patients with atrial fibrillation (479.86±216.82 pg/ml) was higher than that in healthy people (20.95±10.75 pg/ml), and the difference was significant (p<0.05).2. The serum level of NT-proBNP in patients before electrical conversion was 479.86±216.82 pg/ml, but one month later after electrical conversion, it measured 372.46±122.23 pg/ml, the difference was significant (p<0.05).3. Among the 80 atrial fibrillation patients,44 were insufficient in diastolic heart function, the other 36 were normal. And their level of NT-proBNP measured 417.6±113.7 pg/ml and 317.3±110.3 pg/ml respectively.4,The serum level of NT-proBNP in patients with insufficient diastolic heart function (589.95±208.45 pg/ml) was higher than that of people with normal diastolic heart function (345.31±137.36 pg/ml), the difference was significant (p<0.05).Conclusions:The serum level of NT-proBNP in patients with atrial fibrillation was higher than that in healthy people, the level in patients with insufficient diastolic heart function was higher than those with normal diastolic heart function and decreased significantly one month after they were given a second electrical conversion. Therefore, with the clinical significance, the serum level of NT-proBNP can be used as an index of evaluation, treatment and improvement of insufficient diastolic heart function in patients with atrial fibrilliation.
