Background Diastolic heart failure(DHF),known as heart failure with preserved ejection fraction(HFPEF),is due to the reduce of compliance of myocardial.The patients could have the symptom of chest tightness,shortness of breath,and nocturnal paroxysmal dyspnea,but their ejection fraction(EF)is normal.A large number of studies show that the ratio of DHF in patients in heart failure rises gradually,and the prognosis of the patients with DHF is same to the patients with heart failure with reduced ejection fraction(HFREF).So it is very important to predict the left ventricular diastolic function.In the clinical,the most common method of examining the left ventricular diastolic function is the echocardiography.The studies about predicting the left ventricular diastolic function are more and more in pulse doppler(PD)and issue doppler technology(TDI).The study was to discuss the diagnostic value of echocardiography techniques in the evaluation of the left ventricular diastolic function of the patients with sinus rhythm and atrial fibrillation and the patients with atrial fibrillation before and after cardioversion.Objectives The study is to discuss the diagnostic value of echocardiography techniques in the evaluation of the left ventricular diastolic function of the patients with sinus rhythm and atrial fibrillation.Methods1.51 patients with cardiovascular diseases were chosen as the research subject,and 50 normal people as the control group,they were checked by color doppler echocardiography in Chinese people's armed police logistics college affiliated hospital.PD were performed to measure early(E-wave)and late(A-wave)diastolic mitral blood flow velocities,and E/A was calculated,TDI were performed to measure early(Em-wave)and late(Am-wave)mitral valve ring motion velocities,Em/Am and E/Em were calculated,and they were compared between the two groups.2.31 patients with paroxysmal AF and persistent AF after catheter radiofrequency ablation or drug therapy.And 33 patients with sinus rhythm were chosen as the research subject.The patients were divided into the group with AF and the group with sinus rhythm.The patients with diseases that have an affect on cardiac function were eliminated.E,Em,the left ventricular early diastolic flow propagation veloeity(VP)derived from the routine PD and TDI.And E/Em and E/VP were calculated they were compared to the group with sinus rhythm.Results1.Compared with the control group,E-wave was significantly decreased(p=0.012),A-wave was significantly increased(p=0.013),and E/A<1(p=0.001)in the research subject,and the differences were statistically significant(p<0.05),Em-wave was significantly decreased(p=0.002),and Em/Am<1in the research subject(p<0.05),the differences were statistically significant.E/Em>8,and the differences were statistically significant(p<0.05).2.(1)AF patients and sinus rhythm patients was not significant difference in the sex,age,blood pressure and deseases(p>0.05).Compared with the sinus rhythm pstients,decrease of EF,Em,VP and increase of E/Em,E/VP of AF patients were significant in statistics(p<0.05).(2)E/Em,VP,E/VP were not significant difference before and after the patients with AF converting sinus rhythm(p>0.05).Conclusion1.Left ventricular diastolic function decreased in the patients with hypertension and coronary heart disease(CHD).And TDI is more accurate and capable of distinguishing pseudonormalized transmitral flow pattern.The combination of them can evaluate the left ventricular diastolic function more precisely.2.There are diastolic dysfunction of LV in the AF group compare with the simus rhythm group.3.E/Em,VP and E/VP could be the index of evaluating the left ventricular diastolic function in patients with paroxysmal AF.
