Objectives To investigate the implementation of the surgical safety checklist in a secondary level hospital in Shandong Province and to improve the situation.Methods 500 cases underwent different surgical procedures from February 2011 to July 2011 were selected randomly in a secondary level hospital in Shandong province, the surgical team were divided into four parts, (surgeon, surgeon assistant, anesthesiologist, scrub nurse). Before induction of anesthesia, skin incision and leaving operating room, the members in surgical team were asked questions one by one by the trained circular nurse according to the surgical safety checklist recommended by WHO and Ministry of Health of the People's Republic of China, and the answers were filled in the form. The consciousness and compliance rates among the surgical team were summarized and compared and the reasons that affected the implementation of the checklist were analyzed respectively.Results The compliance rates were different among the surgical teams, surgeons was 67.80%, surgeon assistant 74.20%, anesthesiologists 69.40% and scrub nurses 75.60% respectively, (χ2=10.348, P=0.016< 0.05).1. The consciousness rates of surgical team were not reached 100% in all items.2. The surgeon was the lowest one among four groups in consciousness rate of the patients' name (71.98%,χ2=47.647, P< 0.01), ID (45.43%,χ2=17.453, P<0.01), age (71.98%,χ2=54.358, P< 0.01), anesthesia informed consent (81.12%,χ2=82.442, P<0.01) and allergy history (50.44%,χ2=213.507, P<0.01), but in the rate of the procedures (99.41%,χ2=142.458, P< 0.01), surgical site (99.12%,χ2=77.808, P<0.01), implants (98.53%,χ2=34.253, P<0.01) and the actual type of the procedure (99.71%,χ2=33.922, P<0.01) 3. The consciousness rates of surgeon assistant on the patients ID number (59.03%,χ2= 17.453, P< 0.01), the operation informed consent (98.38%,χ2=112.724, P<0.01), the name (89.76%,χ2=101.117, P< 0.01) and dosage (88.95%,χ2=178.268, P< 0.01) of prophylactic antibiotics and blood prepared before operation (99.46%,χ2=107.406, P< 0.01) were the highest one.4. The consciousness rates of anesthesiologists on the name (56.48%,χ2=101.117, P< 0.01) and dosage (42.94%,χ2=178.268, P< 0.01) of prophylactic antibiotic were the lowest, but in patients'age (90.78%, χ2=54.358, P<0.01), preoperative comorbid diseases (93.08%,χ2=173.591, P <0.01), anesthesia informed consent (99.14%,χ2=82.442, P<0.01), the type of anesthesia (97.41%,χ2=100.431, P<0.01) and the blood transfusion (99.42%,χ2=18.404, P< 0.01) were the highest.5. The consciousness rates of scrub nurses in the name of the procedure (76.98%,χ2=142.458, P< 0.01), surgical site (84.66%,χ2=77.808, P<0.01), operation informed consent (76.46%,χ2=112.724, P< 0.01), preoperative comorbid diseases (62.70%,χ2=173.591, P<0.01), the type of anesthesia (79.37%,χ2=100.431, P<0.01), while blood prepared for the operation (81.75%,χ2=107.406, P< 0.01), implants (87.04%,χ2=34.253, P<0.01), the blood transfusion (94.74%,χ2=18.404, P< 0.01), the type of the procedure(91.53%,χ2=33.922, P< 0.01) were the lowest one, while on the patients' name (89.15%,χ2=47.647, P< 0.01) and allergy history (88.89%,χ2=213.507, P<0.01) were the highest one.ConclusionsThere were considerable poor appliances in surgical safety checklist among the surgical team in the secondary level hospital. It was difficult to achieve complete consciousness of the item in the checklist. It should be improve the situation by team member cooperation and collaboration.
