Background Septoplasty is a common disease, and surgery is the only way to solve the septum. The traditional surgical approach is under microscopy to correct the septum. This surgical procedure was limited by a single incision. Recently, more and more surgical procedure were taked under endoscopy, which modifies surgical incision and greatly expanded the septum of the surgical indications. More clinical data needs to comfirm the surgical way for nasal septum incision.Objective To compare the effects of endoscopic surgery with modified incision on nasal septum deviation with traditional surgery.Methods 48 patients with congenital nasal septum deviation were treated by endoscopic surgery, and 45 cases were treated with traditional surgery. The incision alteration, operation time, bleeding, therapy effect and complications were analysised statistically after operation.Results In traditional group, the mean operative time was 87.01±6.13 min, the loss of blood during operation was 13.9±2.17 ml, and the average day in hospital was 6.6±0.82 days.30 cases were cured in 45 patients and the cure rate was 66.7%.12 patients were improved and the improvement rate was 26.7%.15 patients were with postoperative adhesions of nasal septum to middle turbinate in 2-4 weeks after the operations, and 12 cases were improved after isolation the adhesion.2 cases appeared nasal septum perforation.1 case was with wound infection. The complication rate was 6.7%. In flexible endoscopic incision group, an average operating time was 63.10±7.11min, the loss of blood was 8.6±1.33 ml, and the average day in hospital was 3.6±0.71 days.46 cases were cured from 48 patients and the cure rate was 95.8%.2 cases were improved and the improvement rate was 4.2%. Only 2 cases were with postoperative adhesions of nasal septum to middle turbinate in 2-4 weeks after the operations, and all was recoevered after isolation the adhesion. No serious complications occurred, such as delayed perforations, nose collapse, hematoma, wound infection, etc. Less pain of patients was in endoscopic group than in traditional group. The difference of operation way in traditional group and the flexible endoscopic group was obvous (P<0.05).[Conclusion] Compared with traditional surgery, endoscopic surgery with modified incision is a better way on the therapy of nasal septum deviations.
